1、兴港公寓大模板安装、拆除工程安全专项施工方案 大模板安装、拆除工程安全专项施工方案赤峰宏基建筑集团、兴港公寓项目部一、 工程概况兴港公寓2#楼，1-2层是公建，3-26层是公寓；结构形式是框剪结构、一层层高3.6米，二层层高3.3米，3-26层层高3.0米，总建筑面积为46824.29。内外剪力墙设计为大模板施工。二、 大模板的设计与制作。 本次施工所使用的大模板是由北京奥宇模板有限公司设计并制作成型，运送到施工现场；经进厂时检验，质量及安全性能满足设计和施工要求。三、 现场人员的组织 施工现场成立大模板安装安全领导小组 组长： 李叶军 副组长 于成海 曲桂栋 蒋兴福 组员： 唐松林 吕宗江
2、张宝成根据大模板施工需要，组织安装工人60名，吊装指挥信号工2名； 四、现场安全技术要求：1、所有参加大模板安装.拆除工程施工人员进入现场必须遵守公司项目部的规章制度，必须通过三级安全教育，遵守安全纪律，戴好安全帽，不许饮酒，上岗前必须开班前教育会，施工人员要着装整齐，高空作业人员要防止高空坠落物体，高空外墙支拆作业时要系好安全带，施工时如发现违章作业，违反安全纪律现象，按有关安全规章及项目部有关规定进行罚款处理。2、吊装模板必须使用带卡环吊钩，当风力超过五级时应停止吊装作业。吊装时设专人指挥，模板起吊要平稳，不得偏斜和大幅度摆动。操作人员必须站在安全可靠处，严禁人员随同模板一同起吊。安装、起
3、吊外挂架时，操作人员必须佩带好安全带。3、拆模时，起吊模板前，应先检查模板与混凝土结构之间所有对拉螺栓、连接件是否全部拆除，必须在确认模板与混凝土之间无任何连接后方可起吊，移动模板时不得碰撞墙体。4、起吊模板前，应将模板操作平台上的杂物、工具等清理干净，各种连接件严禁钩挂在模板上随模板一同吊走，防止脱落伤人。5、在电梯间进行模板施工时，必须逐层搭设安全防护平台，层层挂设安全网。电梯门、外墙落地门等处要做好封闭防护。6、根据设计计算，模板单重约为125130/；在起吊前必须消除模板与混凝土面的吸附力。五、 大模板的计算见设计方案六、 大模板工程质量技术要求：a) 施工技术人员要根据单位工程模板的
4、特点作好模板施工作业方案。b) 施工前班组长对班组施工人员做技术交底，明确模板施工顺序以及施工方法，做好施工前应具备工作的电力，工具，机械保护用品及人员等准备。施工时班组长必须监守岗位亲自指挥，不准脱离现场。c) 大模板施工质量要求，大模板拆除后里外墙不抹灰，墙面按照国家验收规范及质量标准执行，剪力墙合模前一定检查底盘线和水平线符合标准，墙根部位是否清理干净，检查无误后开始合模。六.大模板施工操作工艺和技术要求： 1.大模板的 各部位的几何尺寸严格按照大模板施工图施工，根据大模板设计方案，以及技术交底要求安装。 2. 施工前，所有墙柱钢筋就位，不得偏差。 3. 按放好控制线安装大模，墙面垂直平

6、砼强度达到允许拆模时，拆除大模进行下一道墙工序。 缝中模板拆完，进行另一道墙钢筋绑扎，钢筋绑扎完焊好支架，缝中填充聚苯板，聚苯板为200厚，进行单面模板安装，浇筑砼。此施工做法保证了墙体施工质量。 八、.墙体模板分项工程施工要点和注意事项： 模板班组应组织操作人员安装模板支腿及操作平台排架支 腿位置应根据模板平面图步排，支腿地脚丝杆应涂润滑油，大模板支腿安装位置在模板主背楞上。操作平台排架安装位置依据模板长度确定，间距不宜大于1800，且安装在距模板边6001000范围内的模板纵向主肋上。 对门窗洞口模板及预埋件进行加固，尺寸，位置进行验收以免会模前或浇砼时位移，将墙体内杂务清理干净。门窗洞口
7、模周边以及导墙部位均应粘贴海绵条，以确保浇注混凝土不漏浆，保持墙面洁净！ 入模安装应先内后外，按施工流水段要求分段或分房间进行，直至模板全部安装就位，在合完墙体一侧模板合另一侧模板前，一定要把固定门窗洞口的扎丝铁线拆除，以防止洞口模板边沿漏浆。 校正模板与安装墙螺栓同步进行，墙的宽度尺寸偏差控制在-2每层模板立面垂直度偏差控制在3范围内。穿墙螺栓的卡头应竖直安装不得呈现水平或倾斜状态，防止脱落，穿墙螺栓安装必须紧固牢靠。用力得当，防止出现松动而造成涨模不得是模板表面产生局部变形，不得漏安穿墙螺栓。 模板安装完后，根部存在缝隙处，需填塞水泥砂浆，但不能进入墙体里面，防止因漏浆而发生墙体烂根，露筋

12、绝缘，漏电保护装置齐全，切实做好模板垂直运输的安全施工准备工作。3、模板工程工作高度在2m或2m以上时，应根据高处作业安全技术规范的要求进行操作和防护，要有安全可靠的操作架子，在4m及2层以上操作是四周应设安全网，防护栏杆。在临街及交通要道地区施工应设警示牌，避免伤及行人。4、操作人员上下通行，应走马道、乘施工电梯或上人扶梯等；不准攀登模板或脚手架上下，不准在墙顶、独立梁及其他狭窄而又无防护栏的模板面上行走。5、在高处作业架子和平台上一般不宜堆放模板，若短时间堆放时，一定要码放平稳，控制在架子或平台允许的荷载范围内。6、高处支模所用工具不用时要放在工具袋内，不能随意将工具、模板零件放在脚手架上
13、，以免坠落伤人。7、雨季施工时，高耸结构的模板作业要安装避雷设施，冬季时对操作地点和人行道的冰雪要事先清除掉，避免人员滑倒摔伤，5级以上大风天气不宜进行大模板拼装和吊装作业。8、在架空输电线路下进行模板施工，如果不能停电作业，应采取隔离防护措施，其安全操作距离应符合施工现场临时用电安全技术规范（JGJ46-2005）的要求。9、夜间施工，要保证足够的照明灯具，照明线路要保证绝缘，并且所有灯具不准固定在模板上。10、模板支撑不能固定在脚手架或门窗等不牢靠的临时物件上，避免发生倒塌或模板位移。11、模板安装过程中，不得间歇；柱头、搭头、立柱顶撑、拉杆等必须安装牢固成整体后，作业人员才允许离开。12
14、、墙模板在未安装对拉螺栓前，板面向后倾斜一定角度并撑牢，以防倒塌。安装过程中随时拆换支撑或增加支撑，以保持墙模处于稳定状态。模板未支撑稳固前不得松开卡环。13、支设悬挑形式的模板时，应有稳固的立足点，；支设临空构筑物模板时，应搭设支架。模板上有预留洞时，应在安装后将洞盖没；混凝土板上拆模后形成的临边或洞口，应按规定进行防护。14、在模板上施工时，堆物不宜过多，不宜集中一处，大模板的堆放必须有放倾措施。15、拆模时必须满足拆模时所需混凝土强度，经工程技术领导同意，不得因拆模而影响工程质量。16、各类模板的拆除顺序和方法，应根据模板设计的规定进行，如无具体规定，应按照先支的后拆、后支的先拆，先拆非
15、承重的模板，后拆承重的模板和支架的顺序进行拆除。17、拆模作业时，必须设置警戒区域，并派人监护，严禁下方有人进入。拆模必须拆除干净彻底，不得留有悬空模板。18、拆模高处作业时，应配置登高用具或搭设支架，必要时系安全带。模板拆除前，作业人员要实现检查所使用的工具是否完好牢固。19、拆模作业人员必须站在平稳可靠的地方，保持自身平衡，不得猛撬，以防失稳坠落。20、作业人员在拆除过程中，如发现已灌注混凝土有影响结构安全的质量问题时，应暂停拆除，报告施工员经过处理后方可继续施工。21、拆除模板一般采用长撬杠，严禁作业人员站在正在拆除的模板上或在同一垂直面上拆除模板。22、严禁用吊车直接吊除没有撬松动的模
16、板，吊运大型整体模板是必须焊有卸甲，必须拴接牢固，且吊点平衡，吊装、运大模板时必须用卡环连接，就位后必须拉接牢固方可卸除吊环。23、拆除电梯井及大型孔洞模板时，下层必须支搭安全网等可靠防坠措施。24、拆除高度在3m以上的模板时，应搭设脚手架或操作平台，并设防护栏杆，拆除时应逐块拆卸，不得成片松动、撬落和拉倒。严禁作业人员站在悬臂结构上面敲拆底模。25、在拆除用小钢模支撑的顶板钢模时，严禁将支柱全部拆除后一次性拉拽拆除。已拆活动的模板，必须一次连续拆除完，方可停歇，严禁留下不安全隐患。26、楼层高处拆下的材料，严禁乡下抛掷。拆下的模板、拉杆、支撑等材料，必须边拆、边清、边运、边码垛。模板拆除后其
