1、预应力混凝土空心板锚垫板张拉破损处理方案一、概况在2014年3月6日上午9时、3月9日下午3时及4时左右对德化门桥梁预应力空心板231、237#张拉，在张拉过程中即发生锚垫板破损现象，造成张拉失败。我公司对事件发生后立即对事件进行了分析并作出了处理方案。二、事故分析事件发生后我公司施工、技术负责人立即会同监理、业主代表一道对空心板锚垫板张拉破损原因进行了以下2个方面分析：1、混凝土外观质量2、从施工记录：从外观上看锚垫板破损处，锚垫板周边是经过修补的，原混凝土与修补混凝土颜色不一；从施工记录查出，在拆模施工中，锚垫板固定在模板上的螺栓未经拆除，而强硬拆模，导致锚垫板被损坏，工人未按当时做出的处
2、理方案而私自进行了处理，从而导致事件的发生。三、处理方案1、先放张锚具卸下工作锚，拆除破坏的锚垫板，在锚垫板损坏处向梁长及梁宽方向分别进行扩大处理，向梁长方向剔凿不少于250mm，梁宽方向每边不少于120mm。2、剔凿后，重新修整钢筋，按规范正确安装锚垫板3、清理干净浮灰后在进行模板安装。4、拌合高强度（C55砼）、早强及微膨胀混凝土进行浇筑，浇筑前用素水泥浆在剔锚处涂刷一层，浇筑时严密振捣并预留试件。5、混凝土浇筑后，确保混凝土养护温度及湿度。四、质量控制 在锚端修复中严格按产品要求配料，在施工同时取制试件，同条件养护，按检测资料分别24小时、3天及7天抗压强度，待强度得到100%后方可进行
