2、报名表中、英文各一份，必须本人签字； （2）提交最高学历证书原件及复印件和有效个人身份证原件及复印件各3份，蓝底彩色近照一寸8张、小一寸白3张； （3）缴纳报名费。 （4）参加入学测试； （5）报名资料经审核符合条件的，发放亚洲城市大学MBA班入学通知书、国际学生卡（含登录学校官网的账号和密码）。6、退款政策与程序 本人在此同意：（1）提交本人的申请表并支付报名费，如本人未能获得亚洲城市大学MBA入学资格，所有费用不予退还；（2）入学通知书中规定的前期费用一律不予退还；（3）如需退款，必须于开学前7个工作日前申请，审核通过后学费的40%可予以退还，违反上述期限规定或未按照规定格式申请者，亚洲城
