11、工程质量，特制定本办法。本办法重点针对特大桥、高架桥、大桥，其它如上跨桥、涵洞、通道等小型构造物参照本办法执行。一、 承包人 1. 承包人：所有施工标段严格按照合同文件的规定和满足施工现场需要的原则来配置人员，包括项目经理、项目总工、专业工程师和各类技术员工，要健全符合要求的工地试验室并取得交通厅质监站颁发的临时资质证书。特大桥项目，每标段应具备46名结构工程师。其学历、职称、实践能力应得到驻地监理工程师的认可。否则，监理工程师应指令承包人增加或调整现场结构工程师。工地试验室应配备46名专职试验员，并有一名精通业务的技术干部担任室主任。各专业班组应视工程实际，配足熟练工人，严禁全部由未经培训的
12、农民工充任。2. 监理：驻地监理组，每个特大桥应至少有一名结构监理工程师。结构监理工程师应委派35名监理员常驻施工工地，负责施工现场的日常监理。二、 机械设备机械设备是保证施工质量的前提。每个施工标段应严格按照合同文件的中所列机械数量和满足施工实际需用来配置，对于不能满足上述要求的，监理组要责令承包人添置或租赁设备，直至满足施工要求。到场的施工机械，除应满足数量要求外，还应保证机械设备的完好率及良好地机械性能。大型、专用施工机械，要实行定机、定人、定岗、定责。施工现场要挂牌施工，标牌有责任标牌、安全警示牌、技术参数标牌。标牌的尺寸、字体应统一、规范。监理组应按照上述要求，逐项检查，对于不合要求
13、的，要下工作指令或暂时停工令，限期整改。对于高标号砼（C40、C50）施工，承包人应自置强制式拌和设备，拌和站要进行达标检查，配料斗至少要有四个（砂1，石子2、水泥1）。粗、细骨料要做筛分和级配试验，粗集料要连续级配。三、 原材料控制所有进场原材料都应符合规范的有关要求。承包人在施工过程中还应注意：1. 钢材、水泥应在业主指定的厂家采购，并向厂方索取质保资料。2. 钢筋进场后，应做抗拉、冷弯、可焊性试验。预应力钢丝、钢铰线参照钢筋试验项目。3. 预应力砼所用水泥，应采用不低于425号的硅酸盐水泥，不宜使用矿渣硅酸盐水泥，不得使用火山灰水泥或粉煤灰水泥。4. 地方材料（指砂、石），不同来源的集料
14、，不得混合储存，更不能在同一标号砼的结构或同一部位的浇筑中混杂使用。5. 模板、支架的制作及质量要求见监字200012号文件。四、 施工准备 1. 各种标号的砼，应在砼浇筑前至少35天完成砼配合比设计试配，要向监理工程师提交各种配合比的全部详细结果，包括强度、各种集料的级配、混合料级配、配合比、水灰比、外加剂、坍落度等，并请监理工程师批准。如欲购商品砼时，应向监理工程师提供生产厂家的各种资料，内容包括厂家地址、供货能力、供货信誉、混合料比例、水灰比、和易性、28天强度等。C30以上标号的砼，严禁使用商品砼。2. 每个分项工程开工前，应向监理组报送开工报告，内容包括人员、机械设备到位情况、原材料
15、及配合比试验、施工组织、质量保证措施等，未经批准的分项工程，不得擅自开工。上道工序未经检查、验收，不得进行下道工序施工。3. 承包人应慎重对待砼试块的制作和标养。砼试块的制作应上振动台振捣，试块应在标准养生室标养，养生温度为203，相对湿度大于90%。五、 工程施工 承包人应严格按照桥涵施工技术规范和省指办有关文件精神进行施工。在保证内在质量的前提下，尤其要注意结构物外观质量，向业主提供一份精美的产品。1. 钻孔灌注桩钻孔灌注桩工作内容包括施工准备、钻孔、清孔、成孔检验、安装钢筋笼、浇注水下砼，截桩头及清除砼、质量检验、低应变检测、钻取芯样等，承包人除应严格按规范施工，做好各工序检查外，在施工
16、过程中还应注意：（1）承包人应根据桥位所处的地质、水文等条件以及桩的尺寸选择适宜的钻孔机械、钻孔方法。钻孔要由有丰富施工经验的施工人员主持，现场应安排专人负责和合适数量的熟练操作工人。（2）护筒、泥浆应按规范要求制作，护筒的位置、长度，泥浆的性能指标，要根据技术规范和现场实际来确定。（3）浇注水下砼，要精心准备，确保万无一失。要具备自发电设施并能保证随时启用。要连续浇注，不允许浇注过程中断或间断过程过长。（4）提升导管时不允许猛提，提升一定高度后应缓慢回落3050厘米，以免钻孔桩取芯时有离析现象。导管埋置深度既不宜过小，也不宜过大，一般应控制在26米范围。（5）要注意环境保护，防止泥浆外溢，以
17、免污染河流及周边环境。2. 下部构造下部构造包括承台（系梁）、立柱（墩身）、盖梁（台帽）。这部分施工的重点是砼振捣和外露面砼的光洁度，在施工过程中，应注意：（1） 模板应采用大块金属模板或高质量的竹胶板。（2） 立柱（墩身）浇筑，应分层捣实。分层厚度不宜过大。（3） 盖梁（台帽）拆模时，应精心操作，防止掉角等现象发生。3. 上部构造（1） 悬浇挂篮a. 连续箱梁的挂篮制作，应进行设计计算并得到监理工程师批准。b. 挂篮应进行试拼，全面检查，发现问题及时处理，组拼后进行预压，以测定挂篮的弹性挠度并消除其非弹性挠度。c. 挂篮行走或浇注砼时的稳定系数，应不小于2.0。d. 严格按设计提供的施工节段
18、按顺利对称、平衡浇筑梁段砼。e. 只有当梁段砼的强度达到设计要求后，才能进行预应力张拉和压浆。（2） 梁体预制a. 预制梁（板）应在符合规范的预制场内集中进行。场地要硬化处理。拌和台、预应力张拉台座要进行达标验收。b. 振捣机械要用附着式、插入式振捣器配合使用。c. 砼拌和、运输、浇筑、振捣等施工细节，承包人要写出书面材料，报监理工程师批准。d. 无论是洒水养生，还是蒸汽养生，都要符合规范要求。（3） 架梁设备a. 架梁设施要进行设计计算并得到监理工程师批准。b. 架梁设备在正式起吊安装前，要进行满载或超载的起吊试验，以检验起吊设备的可结性，进一步完善操作方法。c. 架梁的现场操作人员，上岗前
19、要进行培训。高空作业时要佩带安全帽、安全带、劳保鞋、工作服。d. 施工现场要设立安全警示标牌，严禁无关人员进入场内。专职安全员和值班人员要坚守岗位。（4） 桥面工程a. 在进行桥面铺装前，承包人应提交一份浇筑顺序，浇筑方法的详细说明，供监理工程师批准。b. 桥面铺装应在全桥宽上进行，严禁半幅施工。c. 在桥面铺装前，其梁板表面必须粗糙，铺装前用水冲洗干净。d. 应使用普通硅酸盐水泥，水泥用量应不小于320kg/m3。e. 桥梁砼护栏采用现浇。六、施工记录 承包人：应认真填写各种施工原始记录，包括施工申报表、开工报告，试验表、检查表、自检评定表，如实反映实际施工状况。不允许编造或事后拼估补填，原
20、件要用总监办统一印发的篮头表进行填写。监理组：认真督促、核查承包人填写的各种记录并加以确认，及时对报告进行批复和转呈，对已完的分项工程进行验收、评定。七、检测与计量1. 检测 施工过程中，严格按照省指办下发的有关检测标准及频率进行检验。分项工程完成后，驻地监理组按公路工程质量检验评定标准和省指办有关规定对该工程进行中间交工验收，并对其质量进行综合评定。2. 计量（1）执行监字200011号文件规定。 （2）需附质保资料（如28天强度报告）的分项工程，当月计量时，可按工程量的80%划入材料预付款项，待强度报告出来合格后，下次计量时，转入600章计量，扣材料预付款。 （3）若分项工程评定为不合格者
21、，承包人应无条件地按省指下发的补救方案施工（费用自付），按期保质完成。省指还将对承包人进行通报批评和经济处罚。八、奖惩1. 评定 在每个施工标段的结构物分部工程完成后，省指将组织总监办、工程部、中心试验室、驻地监理组，对其质量状况进行评定。评定的依据严格按照合同文件、技术规范和省指办有关文件进行。分部工程中的分项工程，合格率必须为100%，分项工程优良率必须90%。达不到要求的分项工程，将责令其返工。对单位工程达到优良等级的，省指办将给予奖励。2. 处罚（1） 监理对承包人： 监理发现施工人员未按施工操作技术规程施工，发现一次，给予警告，并责令其整改；发现二次，给予承包人500元罚款；发现三次
22、，给予承包人1000元罚款。 在施工过程中，监理发现承包人的质检人员不在现场，发现一次，给予当事人警告；发现两次，给予承包人500元罚款；超过两次，将责令承包人调换该质检人员，情节严重者，可将该质检人员清除出场。 在施工过程中，监理发现施工现场存在质量问题，除责令其无条件返工外，还将对承包人进行经济处罚。对一些重要部位的质量问题，如断桩、短桩等现象，除责令承包人自费进行技术处理外，还要承包人写出书面检查，提出整改措施。 承包人在开工报告、工序检查、验收等环节，应严格按有关程序要求进行申报、批复。若违反有关规定，监理应对承包人进行教育、帮助、指导，违反三次以上者，将对承包人进行经济处罚。每次罚款
23、5001000元，情节严重者，可暂停计量。（2） 省指办对承包人： 省指办在巡视过程中，发现承包人的施工现场存在下述问题：a. 未按施工操作技术规范施工；b. 施工机械状况较差；c.质检人员不在现场；d.存在安全隐患，都将对承包人进行经济处罚。发现一次，给予警告；发现两次，罚款1000元；发现三次，罚款2000元，并对承包人通报批评。（3） 省指办对监理： 省指办在巡视过程中，若发现旁站监理人员不在现场，发现1人/次，给予当事者警告；发现2人/次，对监理组罚款500元；发现3人/次，对监理组罚款1000无，并对监理组进行通报批评，对同一人发现三次，将责令当事人退场。 省指办人员在巡视检查中，若
24、多次发现承包人施工中存在问题，但监理尚未发现或发现后未采取有力措施，省指办除对承包人处罚外，还将对监理组、有关监理人员采取相应处罚。 省指办若发现监理组已验收合格的分项工程出现质量问题，发现1人/次，对其专业工程师给予警告；发现2人/次，对监理组罚款1000元，发现3人/次，将责令监理组退场本办法自下发之日起施行。二000年六月十六日proved that a people and a nation, if it does not have its own spiritual pillar, there is no unity of spiritual support, it means th
