11、ting, the way we take surveys solicited a views, combed, related to team building维修、拆除、清淤、绿化及临时性等 工程竣工验收管理办法第一章 总则第一条 为规范平朔煤业有限责任公司（以下简称平朔公司）维修、拆除、清淤、绿化及临时性等工程的竣工验收，在中煤平朔煤业有限责任公司建设项目和工程竣工验收管理制度的基础上，特制定本管理办法。第二条 必须严格按照国家有关部门下达的相关政策、法规、条例和中煤集团公司规定执行。 第二章 验收范围 第三条 维修、拆除、清淤、绿化、临时性工程包括以下内容： 一、在竣工交付使用的建筑物
12、、构筑物上进行的土建或设备的保养、维修、更换、装饰、加固等工程量或工作量较小的维修工程；二、建设部颁布的房屋修缮范围及标准中所述的大修、中修、小修和综合维修工程；三、建筑物和构筑物全部或部分拆除工程；四、河床、蓄水池、排水沟、管道等清淤工程；五、绿化；六、临时性工程；七、其他小型工程。第三章 竣工验收条件第四条 工程已按照施工图（方案）和合同规定的要求全部施工完成，并达到合同约定的使用功能。第五条 施工单位申请竣工验收的合同工程，必须经过监理单位和工程管理部门的审核和确认，并出具工程完成情况表（附表1）。第四章 竣工验收程序第六条 合同工程完工后，施工单位向监理单位（工程管理部门）申请验收，监
13、理单位（工程管理部门）组织工程管理、设计、质监、接收等相关部门对工程进行验收。第七条 验收合格后，监理单位（工程管理部门）向工程管理部正式上报合同工程“验收报告”、竣工资料和工程完成情况表。第八条 工程管理部接到监理单位（工程管理部门）上报的合同工程“验收报告”、竣工资料和工程完成情况表后，及时协调办理维修、拆除、清淤、绿化及临时性等工程竣工表（附表2）和建筑工程保修证书（附表3），并完成竣工结算手续。如果工程实体因拆除等原因而不存在，则建筑工程保修证书不需办理，可以全额结算。第九条 合同中没有约定监理单位的工程，施工管理工作全部由工程管理部门负责。第五章 竣工资料第十条 鉴于此类工程施工场地
14、分散，不能按照建设工程划分单位工程，不构成分部、分项工程，不能形成完整的分部、分项检验批质量验收记录资料，竣工资料不能完整编制，需要工程管理部门、监理单位、质量监督站做好工程过程的现场管理。工程管理部门、监理单位对工程的质量、进度、投资进行控制；认真执行国家颁发的工程建设质量管理办法和平朔公司制定的施工管理办法和规定，严格执行日常的材料检验、隐蔽工程验收、旁站监理制度，做好中间、竣工验收记录。第十一条 施工单位收集、整理施工过程中的施工合同、开工报告、审批设计（施工）方案、材料质保资料、设计变更、隐蔽工程、工程中间验收记录、工程竣工验收记录、工程竣工图纸、文件等重要资料，参照平煤基字20082
15、72号中煤平朔煤业有限责任公司建设项目和工程竣工文件资料管理制度编制工程竣工资料，并由监理单位和工程管理部门审查。第六章 竣工结算第十二条 施工单位依据维修、拆除、清淤、绿化及临时性等工程竣工表、建筑工程保修证书和工程完成情况表可以办理竣工结算手续。工程验收部分共3张表：一、维修、拆除、清淤、绿化及临时性等工程竣工表。工程竣工表是办理合同工程验收手续的主要证件，内容包括工程名称、工程造价、工程简要内容等，由施工单位、监理单位、工程管理单位、工程管理部、规划发展部、建设单位分别签字盖章有效。二、建筑工程保修证书。建筑工程保修证书是按照建筑安装工程管理条例的有关规定和施工合同的规定办理的工程保修书
16、，由施工单位、监理单位和工程管理部门签字有效。三、返回工程质量保证金申请书（附表4）。单位工程保修证书规定的保修期满，没有发生任何质量问题，填写返回工程质量保证金申请书，经保管使用部门和平朔矿区质量监督站分别签署意见，建设单位主管审查批准后，财务部门按规定支付工程质量保证金。第七章 附 则第十三条 本管理办法自发布之日起实施。第十四条 本管理办法由工程管理部负责解释。 附表：1、工程完成情况表 2、维修、拆除、清淤、绿化及临时性等工程竣工表 3、建筑工程保修证书 4、返回工程质量保证金申请书fitted with exhaust started to thermal cracking.
