1、 城市文化景观大道建设用地范围内国有土地上房屋拆除工程 安全施工组织设计 编 制 人： 审 核 人： 编制单位：达州市盛平房屋拆迁有限公司编制日期： 本拆除工程主要为华蓥市原人大、政协办公楼及部分住宅楼，最大高度18米，框架、砖混结构，拆除面积9800。第一章 安全生产规定第一节 项目安全方针、目标项目安全总方针：安全第一、预防为主落实责任。项目管理目标：杜绝重大伤亡事故、因公死亡责任指标为零；因工负伤频率千分之六以下，其中重伤事故频率万分之四以下；杜绝重大机械事故；无环境污染和噪音扰民；无重大违法犯罪案件；无火灾事故。第二节 适用范围主题内容本施工组织设计是针对本单位、本工程安全生产状况，以
2、及所存在的突出问题和薄弱环节，从技术、管理、执法等方面有针对性的对本项目在安全工作方针和安全目标做出的具体规定和采取的措施。 第二章 安全组织机构及管理职责第一节 安全管理组织机构及管理体系 按照安全管理体系建立相应的安全责任制（见下图）。1在公司安全负责人的领导下成项目经理部安全生产领导小组、由项目经理主持安全生产领导小组开展工作，行使职责。2安全生产领导小组日常工作由项目经理部安全负责人、负责。3安全生产领导小组每周召开安全例会，贯彻上级安全生产重大决议和要求，分析研究本工程安全生产形势和现状，制定和审定有关安全生产重大措施，审议安全生产工作专项报告，并做出相应记录。第二节 安全管理职责1
3、项目经理：为项目安全生产第一责任人，对项目安全生产负全面领导责任。领导制定并组织实施年度及阶段安全生产目标与规划，对安全专职机构设置与人员配备负责。2公司安全负责人：协助项目经理全面负责项目经理部安全生产工作，保证安全技术措施费用及时到位，按规定审批各项劳动保护费用和安全生产奖励资金。领导制定并组织实施阶段安全生产目标与规划，决定和批准安全管理工作重要活动及重要决议、决策和文件的制定、贯彻与执行。3现场技术负责人：对项目安全技术管理负领导责任，组织领导各项安全技术措施的编制、审定和批准，使其符合有关安全规程和技术规范，保证施工的安全性。领导新工艺、新技术、新材料开发、应用的安全技术配套管理。4
4、生产执行经理：协助项目经理实施对经理部安全生产工作的管理，实施全面安全管理工作，是本项目安全生产的直接责任者。负责贯彻执行安全生产方针、政策和法规。5项目副经理：负责合约、报价等系统贯彻落实安全生产相关职责，按国家有关部门规定审核准予分管专业项目部安全责任与义务。6项目安全负责人：在现场经理的直接领导下履行对项目安全生产管理和监督职责，宣传贯彻安全生产方针、政策和规章制度，推动项目安全保证体系的运行。对项目各项安全管理制度的贯彻与落实情况进行检查和具体指导，及时发现薄弱环节或失控部位，向施工组及时下达整改指令书，并跟踪复查，督促如期整改；组织专业队伍安全专兼职人员开展安全监督与检查工作；查处违
5、章指挥、违章操作、违反劳动纪律的人员，对重大事故隐患采取有效的控制措施，必要时可采取局部停产的强制措施；做好安全相关资料的记录、管理、备案工作。每月召开一次安全讲评会，分析与总结本月安全生产情况；参与安全事故的调查与处理；对进场专业项目部队伍的三级安全教育，并负责考核、办理与发放安全上岗证；定期或不定期对现场使用中的劳动保护用品、护具抽样检测。在每月底对本月安全生产动态，存在问题、解决方法与下月安全防范重点以书面形势报项目经理、项目副经理。并做好本工程安全设计的编制工作。7施工组：每周配合安全部进行现场安全检查；负责贯彻执行安全生产方针、政策、法规、施工组织设计及安全设计所明确的安全技术保证措
6、施；定期组织对施工现场文明施工进行检查，对发现的安全隐患及安全部所发安全隐患整改指令书按要求时间及时整改。管理和指导专业项目部队伍认真履行安全生产职责，搞好现场安全防护与管理。认真做好各分项施工的安全技术交底，以及相应季节安全技术交底。第三章 安全生产具体管理点1贯彻执行公司的安全管理制度，完善安全管理体系，落实安全生产各项管理制度，确保安全生产。2安全生产管理实施分级管理、分层负责、预空预防的原则，各级各类人员必须履行自己的职责。从项目的策划开始就建立以项目经理为第一负责人的项目安全生产、文明施工管理责任体系，并不断促其有效运行。并将所有专业施工队伍纳入项目的安全生产管理体系中，使安全责任落
7、实人。3坚持安全生产教育培训考核审核制度重点抓好三级安全教育和特殊作业人员的教育培训取证及项目经理部自身管理人员安全资格年审工作。4发挥安全技术措施在安全生产、文明施工方面的保证和促进作用。所有施工必须遵循经批准的施工组织设计（或方案）。施工组织设计（或方案）中必须有针对本工程特点的能保证施工人员安全的安全技术措施贯穿施工全过程的各个环节。5加强对下设施工组的管理，高度重视安全生产施工协议书（责任状）的签订及管理。6严格审查分包队伍的资质，凡进入本工程的分包队伍人员和机械设备必须是合格的、是符合有关标准和法规要求的，其素质与数量必须满足现场安全生产的需要。7特种作业人员必须持有合格有效证件。8
8、在分项工程具体实施前必须签订有明确的安全生产、文明施工、消防管理目标和管理责任划分条款的经济合同（或含有上述内容的协议书、责任状）。9必须贯彻执行安全生产、文明施工法规，必须认真执行安全生产责任制，必须尊重并服从项目经理部对施工现场的安全生产管理。10加强经理部监督管理力度，对严重违章指挥、违章作业、违反劳动纪律者进行必要的处罚及曝光。11严肃事故报告制度及事故现场的保护制度，以及紧急意外事件的应急处理。第四章 安全施工的重点控制措施脚手架根据现场实际情况编制施工方案，并应有设计计算书，根据施工方案搭设，在脚手架设前办理交底，完成后办理验收手续。立杆基础应平实，立杆底部应有底座、垫木和扫地杆。
9、脚手架与建筑结构每层设置拉杆，脚手板须满铺，施工层设1.2m高的防栏杆和不低于18cm的挡脚步板。1逐级进行安全技术教育及交底，落实所有安全技术措施和人身防护用品，未经落实不得进行施工。2高空作业所需料具，设备等根据施工进度随用随运，禁止超负载乱堆乱旗。3高空作业人员须经过专业技术培训及专业考试合格，持上岗并须体检合格。4高空作业人员所用的工具应随时放入工具袋内，严禁高空相互抛掷，传递。7在进行上、下立体交叉作业时首先必须具有一定左、右方向的安全间隔距离，不能确实保证此距离就应设能防止附物伤害下方人员的防护层。8进入施工现场的所有人都必须按规定佩戴安全帽，施工作业层的外侧采用密目安全网封闭，高

11、运至指定地点，严禁将施工弃土、建筑垃圾任意倾倒，生活垃圾必须倾倒入指定垃圾场内。4噪音的控制：施工时尽量避免夜间施工，并采取隔音措施。5选取素质较高的工人，入场后加强教育。第二节 消防保卫措施各施工现场实行总承包负责制，总包单位要对施工现场消防工作负全责，各专业施工队伍服从项目部的管理，双方要签定消防安全协议书，明确双方的责任。实行逐级防火责任制。项目经理是现场防火工作的负责人，并配备消防干部，负责日常消防工作。组建现场的防火领导小组，成立义务消防队，并制定灭火作战计划，建立健全各岗位的防火管理制度、措施，并按要求上墙。专（兼）职防火人员，要做好防火负责人的参谋，组织编制、制定、完善施工现场有
12、关防火安全的规定、规章制度，对现场进行防火安全监督、检查、落实责任，解决隐患。做好宣传教育工作，人员入场前应进行三级教育，组织义务消防队员进行教育，使他们掌握防火常识，训练他们扑救初期小火的技术能力。施工现场禁止非生产人员在现场住宿，工程内住人应由现场防火负责人批准，并报消防监督机关备案。新建建筑物内禁止设库房，不准存放油漆、烯料、石油、液化气、电热器具。如需置，必须上报保卫部；在施工地下室，不准住人和存放易燃、易爆物品。第3节 防止扬尘污染 严格遵守华蓥市关于进一步加强建筑施工工地扬尘专项整治实施方案的规定。1拆除的建渣采用串通向下运输，严禁随意高空抛撒。2每天拆除前，对被拆除的的部分用水湿
13、润，避免因拆除产生扬尘。3、对已拆除但还未运出现场的建渣用太阳膜覆盖。4、对运渣车辆实行全封闭。5、施工现场出入口设置车辆冲洗设备，凡出场车辆均冲洗，并建立冲洗台账。6、施工现场出口道路每天派专人保洁。第四节 防止噪音污染在施工过程中应尽量减少扰民的噪音，对容易生产噪音的工序，采取措施，降低或冲减噪音声源。现场运输车辆出入施工现场禁止鸣笛，对建渣的装卸，尽量减少碰撞声。如需夜间施工的，则要提前对外张贴安民告示用以对附近居民告知，取得附近居民的谅解。第五节 现场临时道路文明施工是一个企业素质的体现，在施工过程中对进出场的车辆要进行文明施工管理，进场车辆所运材料要堆放牢固，不超高、不超载、不违章，
14、确保安全，出场车辆要对车身进行冲冼，不带泥砂出场。施工现场临时道路利用原砼院坝，无需再硬化。现场组织专门力量对临时道路进行维护、清扫、冲洗，确保路面无遗物，无灰尘。第六节 其他1门前实行“三包”，保证现场各类材料堆码有序，现场排污水沟处于良好状态。2施工工人操作地点和周围必须清洁、整齐、做到干活脚下清，活完场地净。3施工机具要做到摆放整齐，机身保持整洁，标语编号明显，安全装置灵敏有效，机棚内外干净。5运输各种材料、垃圾等有遮盖和防护措施，防止泥浆等随车带出场外，影响市容环境卫生。6现场施工人佩戴身份卡。7施工人员进行文明、社会道德教育。要求着装整洁，讲卫生，不随地吐谈，不大声喧哗，严格遵守社会
