1、桂来高速公路5标项目部技术交底 第 9 页 共 9 页 记录表01 施工技术交底记录表承包单位：湖南路桥建设集团公司 合同段：5监理单位：广西八桂工程监理咨询有限公司 编号：工程名称路基边坡开挖施工桩号及部位K1348+300K1348+600交底内容： K1348+300K1348+600路基边坡开挖施工方案一、施工内容待开挖边坡已全部被灌木丛覆盖，安全稳固，为进行桩基础施工，已进行边缘小部分土方开挖，见下图。 边坡正面图 边坡侧面图边坡高8米左右，坡度约为1:1.5，原有浆砌片石骨架防护构造物，由于该地区降雨丰富，因此植被茂盛，边坡已全部被灌木丛覆盖，中间夹杂有35米高的乔木，截水沟已被灌
2、木丛覆盖，截水状况良好，现拟进行覆盖灌、乔木砍伐及清表工作。二、公路运营安全防护措施1、设置砂袋墙（见下图）根据我项目的施工条件，结合现场实际调查，为保证K1348+300K1348+600路基边坡开挖施工过程中不影响原柳南高速公路的正常通车，避免在施工过程中对原柳南高速公路正常通车造成安全隐患，对于砍除原挖方边坡的树木采用在原高速公路防撞护栏外侧堆积砂袋墙的安全防护形式，砂袋墙高1.5，宽0.5米。靠路面这一侧采用高2米的蓝色彩钢瓦包边。防护横断面图彩钢瓦挡板示意图砂袋墙平面示意图2、砍筏边坡树木在砍筏中应采取绳拉或控制刀口方向等措施使树木不倒向公路路面方向，砍伐过程中严禁乱丢烟头，防止发生
3、火灾。3、设置防落网（见下图）开挖边坡时，在原有边坡修筑2米平台，每隔4米安装一处钢管三脚架，上铺防落网，以防止开挖过程中土石块滑落到原高速公路上。在路基坡顶挖方施工时，紧邻老边坡位置预留1米宽不开挖，待分层挖深12米时，再由外向内开挖原预留的1米，减少落土石的几率，同时及时检查开挖后砂袋墙，对松动的土石及时进行清理。防落网示意图4、开挖前先进行灌木、乔木砍伐，再进行清表，先进行A、E匝道线路开挖，待桩基础完工后再进行D、H匝道线路开挖，开挖前应设置临时排水沟，防止冲刷的泥土污染高速公路路面。三、一般施工安全技术措施一般施工安全技术措施1、施工机械作业时，除按规范操作外并应按事先设计的行走路线
4、进行，其工作位置应平坦稳固，并应有专人指挥，指挥人员不得进入机械作业范围内。2、挖方边坡实行“谁开挖、谁加固、谁保护”的原则，施工时严格按照设计方案进行施工。3、边坡施工作业人员必须戴好安全帽，系好安全带，绑挂安全带的绳索牢固地拴在可靠的安全桩上，绳索应垂直，不得在同一个安全桩拴2 根及以上安全绳上拴2人以上。4、边坡施工应设置安全通道；开挖工作面应装运作业面相互错开，严禁上、下交叉作业，边坡上方有人工作时，边坡下方不准有人停留或通行。5、清理边坡上突出的土石和整修边坡时，应从上而下顺序进行，坡面上的松动土、石块必须及时清除。严禁在危土石下方作业、休息和存放机具。6、施工中如发现山体有滑动、崩
5、塌迹象危及施工安全时，应立即停止施工，撤出人员和机具，并报告监理办和指挥部处理。7、滑坡地段的处理，应从滑坡体两侧向中部自上而下进行，严禁全面拉槽开挖。施工中要设专人观察，严防塌方。8、遇有大雨、大雾及六级（含六级）以上大风等恶劣天气时，应停止作业。边坡路堤下方道路，施工时应设置警示标志。9、施工机械靠近路堤边缘作业时，应根据路堤高度留有必要的安全距离（距离边缘1米以上），并设专人指挥，指挥人员不得进入机械作业范围内。10、弃土下方和有滚石危害范围道路，应设警告标志、作业时提醒下方车辆通行。四、安全施工专项措施安全施工专项措施1、必须按设计规范施工。2、全部作业过程严格执行操作规程。3、路基施
6、工准备（1）、机械设备配备：挖掘机2台、装载机1台、推土机1台、压路机1台、平地机1台、自卸翻斗车16台、供电设施一套、照明设备一套、汽车吊1台。（2）、安全防护用品：安全帽、警示牌、口哨、指挥旗、标志牌、警示灯、安全带等。4、开工前检查：挖掘机、装载机、推土机、压路机、平地机、自卸翻斗车、供电设施、照明设备、汽车吊等是否处于良好状态，各项制动是否有效，电缆线有无破损等情况。5、施工安全注意事项（1）、在靠近高速公路交通繁忙地段施工，需安排专人进行警戒。（2）、车辆通过较多、运输较繁忙的便道，弯道半径小于15m，特殊地段小于10m须挂安全隔离标志带及警示牌。（3）、施工现场便道急弯处、陡坡地段
7、要求悬挂标志标识牌。（4）、严禁穿硬底、带钉子、易滑、高根、拖鞋或赤脚进入施工现场。（5）、施工现场材料、设备摆放整齐有序。（6）、施工翻斗车不能行车载人及超载超速。其它施工机械不得超速行驶及违章作业。前后两车（机械）间距不应小于10m。（7）、路堑开挖严禁采取掏底开挖（忌挖神仙洞），以免坍塌。（8）、严禁在松动危石下、未熄火的大型机械旁作业和休息。（9）、弃土场的选址需避免泥石流沟。（10）、严禁在山坡上同一地段的上下不同时作业。（11）、人工挖掘作业人员横向间距不应小于2m，纵向间距不应小于3m。（12）、边坡开挖须从两侧向中部自上而下开挖，禁止全面拉槽开挖。（13）、边坡开挖施工前必须做
8、好边坡截水沟，防止雨水对边坡的冲刷，造成边坡土石方滑溜，对作业人员造成危险和影响既有线的行车安全。（14）、边坡开挖后要用彩条布等及时覆盖，防止雨水冲蚀，坡体失稳坍塌。并确保运营中柳南高速公路两边水沟的排水畅通。在反向开挖、砍除原边坡防护树木的同时，杜绝树木滑落到原高速公路上，局部有危险段或者开挖不到得位置处，采用人工拆除，临时搭建升降机，运送拆除后的树木，对于砍除后的树木应集中堆放后，就近运至桂来高速公路K233+400处进行处理。五、安全生产组织机构及领导小组1、组织机构安全组织机构（二）领导小组（含联系方式）组 长：雷荣华（18674896246）副组长：成 晓（13786262808）

10、应对各种安全突发状况；李征斌（13755169908）为现场施工负责人，负责现场的施工组织工作，并协助刘国强做好安全管理工作。六、突发事件应急处置措施根据挖方段现场实际情况，可能发生的突发事件有火灾、交通事故等。1、火灾应急处理措施 (1)任何人发现火灾时，应立即报警，报警人员在报警时应同时说清着火地点、部位、燃烧物品、火灾状况等。同时报告所安全领导小组。通知相关部、室负责人，同时做好灭火前的必要准备工作，及时记录火灾情况。(2)接报告后，所有义务消防队成员必须无条件及时赶赴现场，参加救火行动。(3)全体在场工作人员、义务消防队成员必须立即加入扑救火灾行动。在灭火救灾指挥部指挥下，疏散救护工作
11、要有条不紊，责任到人，由各安全员负责进行引导护送，向安全区疏散，同时要查清现场是否有遗漏人员。接应救护车，指引救护人员及时对伤员进行处理或送医院救治。(4)现场成立灭火救灾指挥部，由工程部长及事发部门负责人组成，工程部长任总指挥。其主要职责是：负责查看火情，掌握火势发展情况，随时向119指挥中心通报火情，根据火情指挥切断电源、可燃性气体源，指挥义务消防队和增援人员灭火，指挥抢救伤员，疏散物资，及时控制火势蔓延。在向119报警后，及时派出人员接应消防队和清除交通通道障碍，疏散围观群众，做好警戒工作。公安消防队到达后，及时向公安消防火场总指挥报告灾情，按统一步骤，组织职工贯彻执行，做好安全工作。(
12、5)现场指挥员有权根据扑救火灾的需要，决定如下事项：使用各种水源，限制用火用电直至停止使用，划定警戒区。(6)根据现场具体情况划分安全警戒线，安全警戒线分为建筑物外围警戒和火灾现场警戒。 外围警戒要及时消除路障，劝阻无关人员、车辆离开现场，维持好建筑物外围秩序，为公安、消防队到场展开灭火创造有利条件。火灾现场警戒要及时指挥疏散人员，看管好抢救出来的物件。2、交通事故应急处理措施应立即拨打122报警电话;市公安交警部门接警后应立即赶赴现场,处理交通事故或车辆抛锚情况应在事故发生地前端100米处设置警示标志,并立即通知公安交警部门对高速公路实行交通管制。应记录肇事车辆的车牌、车型、颜色及驾驶员的姓
