29、方案。截止2014年12月24日，施工人员、机具等已全部到达工地。为保质、保量、按时、安全完成施工任务，特编制旬邑柏岭寺水库混凝土重力坝廊道排水孔及固结（接触）灌浆施工组织方案。一.编制依据本次施工组织的编制依据为：1. 咸阳市水利水电规划勘察设计研究院咸阳市柏岭寺水库坝基固结灌浆设计图2014年12月，电子版。2.西安方力水电工程有限公司咸阳市柏岭寺水库工程地质补充勘察报告2012年11月。 3.业主方有关领导的指示精神及要求和2013年我公司固结灌浆试验等各种技术参数。 二.工程概况旬邑柏岭寺水库位于旬邑县，坝址西南距旬邑县城直线距离约13km。水库工程位于211国道和306国道的交通线上
30、，在建的西安平凉铁路和银川武汉高速公路自城区边缘通过，县城至水库坝址施工处有乡村公路相通，交通较为方便。截止2014年11月，旬邑柏岭寺水库工程已完成右岸土坝、右岸混凝土挡土墙、土坝混凝土截流墙、左岸平洞等分部工程施工，混凝土重力坝现已浇筑至1053m高程，因进入冬季而停工。根据业主的统筹安排，为不影响来年大坝混凝土浇筑工程，业主决定利用现空闲时间对廊道内排水孔、混凝土坝体进行接触（固结）灌浆施工，以保来年主体工程如期开工。 三.工程地质简况本节仅对本次施工有关大坝坝基地质归纳简述如下：坝基地质为：白垩系下统志丹工群洛河组（Klz2）：主要分布于坝址区1102m高程以下。岩性为紫红色厚层状中粗
31、粒砂岩夹23层砾岩。砂岩较疏松，具巨大交错层理，砾岩为钙质胶结。厚度约200m。坝基裂隙主要以水平层面裂隙为主，陡倾裂隙发育较少。补勘钻孔岩芯多呈饼状，即：水平裂隙较为发育的反映，岩饼最小厚度小于1cm，说明河床以下局部岩体应力集中，致使水平裂隙较为密集，透水性较好。四工程任务及要求1.廊道排水孔。大坝设有上游廊道和下游廊道，上下廊道纵向长度均为92.09m。上下游廊道底部高程分别为：1035.0m、1035.3m。上下游廊道纵向断面分别为：宽高3m3.5m、2.5m3.0m。上下游廊道排水孔均为斜孔，顶角10.o。上游孔间距为2m，孔深16m，布47孔，合计752m。下游孔间距为4m，孔深1

35、泰安同志任组长的全面质量保证体系TQC领导小组，其人员组成及各级人员职责是： 1组长职责：吴泰安负责排水孔、固结（接触）灌浆全面工作。负责本次工程施工合同的签订与实施。负责公司对外的业务联系，处理有关业务事宜；负责施工人员的组织安排。负责协调公司内部各部门之间的配合工作。负责每周召开一次TQC小组会议，讲评工程进度、质量及注意事项等。2.工程技术负责人职责：方 超对项目经理负责。负责工程的技术工作，及时处理施工现场需要解决的技术事。负责及时进行工程测量与放线工作，对其质量负责。负责安排工程流程，及时协调各流程之间联。负责统计工程进度情况，每天10:00向项目经理汇报前日完成的工程量及需要公司解
36、决的事。负责施工资料的编写及上报工。负责施工质量的把关检查工作。负责施工中的技术安全（地下管线）工作，适时提出安全防护措施，并逐项落实。3.施工现场负责人职责：王延安 车鹏军对项目经理负责。负责工程施工现场的管理。负责施工现场的安全工作，做好安全预防措。负责施工各班施工的数、质量检查核对工作。负责现场施工材料的调配和管理工。负责现场施工中出现的各种技术事宜，并及时与技术负责人沟通。负责完成项目经理交给的其它工作。 4.采购及安保负责人：王延安 吴立成对项目经理负责。负责工程现场安全保卫的总体工作。负责施工放线前地下管线及设施的核对工作。负责施工前对全体施工人员的安全培训教育工作。负责对施工人员
37、的日常行政管理工作。负责水泥等材料的采购工作，做好材料的质量鉴定，提交鉴定文件，保证材料质量优良、证件齐全。负责职工食堂的卫生检查工作，防止食物中毒。负责及时支付各项工程材料款，保证工程资金安全。完成项目经理交给的其它工作。8 施工地场地安排及要求 1.机具100型轻型钻机及钻杆等直接用吊具放至廊道内安装调试；KJ580型大型钻机及钻杆等用25t大开支吊车吊放至大坝右侧施工场地上安装调试，并做好一切开工准备工作。2. 浆液搅拌机具 浆液搅拌机具放置在库内右侧靠大坝15m内，并对场地进行平整夯实。3.水泥堆放水泥堆放在浆液搅拌机具靠上游处，距机具12m处，以便于施工。水泥堆放时要做到码放整齐，底
38、部要铺设防潮布，上部用彩条布盖捆，以防受潮受损。以上场地约100m2。4. 施工用水施工用水采用潜水泵供水。潜水泵放至库内临时围堰上游抽水，用高压软管引至施工现场。9. 施工质量控制措施及工艺流程 本次工程施工分两个单元，一是廊道排水孔及反滤材料充填；一是岸坡固结（接触）钻孔灌浆。其质量控质措施及工艺流程如下：1.钻孔质量控制（1）斜孔。排水孔设计要求顶角为10O，为保证斜孔角度合格，拟采取如下3种措施：A.在100型轻型钻机机腿下加加工一三角形底座使钻杆角度为10O；B.校正施工场地水平面，不平时用木板支平；C.在孔深至10cm、20cm、30cm、40cm、50cm、70cm、100cm时
39、分别用三角原理测一次孔斜，并及时校正，使钻孔从始至终保持在10O1o（16m孔端底偏差30cm、10m孔端底偏差17.5cm）。（2）灌浆孔。固结（接触）灌浆孔钻孔时，首先要保证钻机处于水平状态，不平时要用木板支平，孔深在50cm内每隔10cm用吊锤测一次孔的垂直度，以保证钻孔端底偏差25cm。 （3）孔深。钻孔深度采用钻杆长度进行计量，并做好记录。2. 固结（接触）灌浆 本次固结（接触）灌浆不再进行试验，直接采用2013年我公司所做的固结灌浆试验所给出的各种参数。其具体参数为：基岩段水灰比1：0.5（重量比）、混凝土段1：0.7（重量比）灌浆自下而上分段进行，混凝土与基岩接触段（1m）灌一次
40、，作为灌浆的重点段，向上混凝土段每5m为一灌浆段。灌浆至孔口段时，待浆液初凝后要进行二次补浆。封孔口采用稠砂浆与坝面抹平。灌浆时应按照先外后里，先围后挤的顺序进行操作。3. 钻孔孔距与排距 钻孔孔距与排距严格按要求参数执行。排水孔：上游孔距2m、下游孔距4m，均为单排。固结（接触）布孔采用等腰三角形布孔，孔间、排距3m。具体实施中按宜小不宜大的原则进行。（1）排水孔。测量放线标注孔位点钻机到位开孔口开钻（孔上部按前述方法测斜度）用高压洗孔充填反滤料至孔口50cm处放置土工布及70mmPVC管充填细砂结束做好记录。 （2）固结（接触）灌浆。测量放线标注孔位点钻机到位开孔口开钻（孔上部按前述方法测
41、孔垂直度）用高压洗孔灌浆二次补浆封孔口结束，做了记录。4.施工质量鉴定（1）排水孔质量鉴定内容分为孔深及孔斜两项。孔深用钻杆长度现场进行检测；孔斜采用三角形原进行检测。（2）固结（接触）灌浆质量鉴定内容孔深及灌浆质量（效果）两项。孔深鉴定方法同排水孔。灌浆质量（效果）采用双孔压水法进行。具体方法为：在右边、左边坝分别进行1组、2组，半3组。每组钻2孔。孔深同所在区灌浆孔相同，选其中一孔进行压水，另一孔观察不冒水时，即可认为固结（接触）灌浆合格。 固结（接触）灌浆质量鉴定应在28天后进行。 十.施工进度控制措施为保证大坝混凝土工程年后如期复工，根据任务紧、要求严、冬季施工难度大的特点，我公司争取
42、本次工程在年内完成。为此采取如下措施：1. 平行作业 平行作业的具体方法为：排水孔与固结（接触）灌浆两单元工程组织两台机组同时分头施工。2. 交叉施工交叉施工的具体方法为：灌浆人员在早上及傍晚可与排水孔反滤人员集中充填排水孔；中午天气好时两班人员集中灌浆。总之，要合理科学安排劳力、机。做到不误时、不误事，达到 缩短总体施工时间、提高施工工效之目的。3. 施工工期安排 施工工期安排详见图二排水孔及固结（接触）灌浆施工进度线状图（见下页）十一.施工安全控制措施1.严格执行国家颁布的建筑施工安全技术操作规程及有关安全生产的规定，利用多种宣传工具和多种形式，教育职工树立安全第一的观念，强化安全生产意识
43、。 2.施工过程中每个质量安全认真贯彻执行国务院建筑工程质量条例，严格按规范、规程验收标准施工进行内部验收。 3.建立健全安全生产责任制，制定有关各种安全技术操作规程。落实到人，责任到人。各施工分队所辖区段安全生15产管理责权明确。4.落实承包责任制。施工现场由施工队长负责安全生产，并设置专职安全员。安全生产是一项重要指标，把安全生产与每个职工的切身利益挂钩，对安全生产中做出突出贡献的职工进行奖励。 5.安全第一，预防为主。配备足够的防护用品，加强劳动保护，开展好“安全周”、“全工施工过程无事故”等活动，坚持每周一安全教育，班前有要求，班后有讲评。每周末进行一次安全检查。每班带班人员，发现问题
44、及时查处，不留隐患。 6.所有施工临时设施必须进行技术交底（如配电设施等），使操作人员撑握操作方法，熟悉各个环节的技术使用注意事项。 7.各类施工机械必须配有安全操作牌和安全保护装置，使所有操作人员都能安全操作、规范操作。严禁非机械操作人员操作机械，严禁违章作业。项目部要定期不定期检查，并进行讲评。 8.按照“三不放过”原则，即：违规操作、不穿戴安全用品、现场生明火。及时发现时处理，使职工吸取教训，防止类似问题再次发生。 9.加强对危险作业的安全检查。施工人员进入施工区时必须佩带安全帽，系牢下颚带。不带安全帽者一律不准进入施工现场。施工中不戴安全帽者，发现一次罚违者款50元。严禁穿拖鞋进入施工
45、现场，更不得裸脚施工。做到安全施工，文明施工。 10.工地机电用具、室外临时电器（含开关）必须设置雨棚，配触电保护器。引接电缆应架空或埋入地下，接头应用绝缘胶布扎好，以防漏电伤及人员。 11.始终保持安全设施完善，安全保护装置完好，安全警示齐全、规范。施工安全保护设施符合规定，防尘、防毒、防害、不扰民。施工现场做到消灭“五头”，即：烟头、废铁头、电线头、铁丝头、焊条头。 12.坚决杜绝食物中毒，不买、不食变质食物，保证职工的身体健康，做到始终保持100%的出勤率。保证工程任务如期完成。 13.所用设备、材料须有合格证。因建材不合格，不管造成质量问题与否，均要追究相应责任人，并责成立即纠正。 1
