1、 北京智博睿信息咨询有限公司 噻托溴铵项目可行性研究报告特异选择性的抗胆碱药物,具有毒蕈碱受体亚型MIM5类似的亲和力,它通过抑制平滑肌M3 受体,产生支气管扩张作用。在临床前的活体外和活体内的研究中显示,本品对乙酰甲胆碱诱导的支气管收缩的阻位点专一制作用具有剂量依赖性并可维持长大24 小时以上。本品吸入后产生支气管扩张作用多半是位点专一效应。临床研究表明,首次给药很30min内能使肺功能得到显著改善,1周内达药效学稳态。本品能显著改善早、晚峰值呼气流速(PEFR)。并且在1年的给药期内一直保持其支气管扩张作用,而无耐受现象发生。此外,还能显著改善呼吸困难。合成的,非手性的四价铵化合物,白色或

2、淡黄白色粉末。微溶于水,可溶于甲醇。青年健康自愿者吸入本品后,绝对生物利用度为19.5%,食物不影响其吸收。在稳态时,慢性阻塞性肺疾病(COPD)患者吸入本品18 mcg,5 min血药浓度达峰值(1719 pg/ml),其后以多室模型的方式迅速下降,稳态时的血药谷浓度为34pg/ml。本品血浆蛋白结合率达72%,分布容积为32L/kg。本品不能通过血脑屏障。本品消除半衰期在吸入很56 d,14%的剂量经尿排出,其余经粪便排泄。本品肾脏消除率大于肌酐清除率,表明药物是分泌入尿液。COPD患者连续每天吸入,23周很达到药动学稳态,其后无进一步的药物累积。存储温度15C28C,勿存放在极端温度和湿
