1、临舍彩钢板房安装安全协议甲方(工程发包方)：杭州萧宏建设集团有限公司乙方(工程承包方)：吴江市震泽万里彩钢板厂 甲方工程由乙方承包施工。为明确双方安全责任，防止乙方施工人员发生事故，经双方协商签订协议如下：一、甲方的安全责任：I、涉及工地临时活动房安装的工作，应派专人进行检查、督促，及时协调和解决施工中出现的问题。2、开工前应对乙方单位负责人和有关工程技术人员进行安全技术交底。3、在安装区域内作业有可能造成生产设备停电、停运事故的，应要求乙方单位事先制订工作的“三措施”(即：组织措施、技术措施、安全措施)，并审查和监督实施。4、对乙方单位负责签发的工作票进行会签。5、向工作班成员交待安全注意事
2、项，施工时严格实施监护。6、应给乙方施工人员办理配有本人照片的“胸卡”证，并在上岗时佩戴。7、不得安排乙方单位人员从事与其承包工程无关的工作。8、工程竣工后，应组织验收，对设备的安全、质量情况作出综合评价。二、乙方的安全责任：1、组织施工人员学习工地临时活动房安装安全工作规程、工地临时活动房安装安全工作规定以及甲方颁发的有关安全生产规程制度，派出的施工人员必须是经甲方安全教育培训、考试(或考核)合格的人员，涉及进入甲方生产、办公场地的施工项目，乙方施工人员应佩戴甲方核发的胸卡。乙方不得派未经甲方安全教育培训、考试(或考核)合格的人员进入施工现场工作。2、复杂或危险性较大的环节，应制订施工“三措
3、施”。经甲方审查合格后贯彻实施。3、工程开工前必须对全体安装施工人员进行安全、技术交底，使全体施工人员均掌握工程状况、特点及施工“三措施”，并对现场施工安全负责。 4、施工中必须严格执行工地临时活动房安装安全工作规程、工地临时活动房安装安全工作规定和甲方单位对安全、文明施工的有关规定和要求。进入安装现场区域内施工，必须按规定严格执行工作票制度。对规定可以不使用工作票的工作，开工前应得到值班人员的同意。5、开工前应对安装施工、工器具及安全防护设施进行检查，确保符合安全规定并不超过检验周期。6、必须按国家规定为施工人员配备合格的职业防护用品、用具；不得使用未成年工和不适应现场安全施工要求的老、弱、
4、病、残人员；对特种作业的工作应派经过有关部门安全技术培训合格的持证人员。7、必须接受甲方的安全监督、管理和指导，企业负责人(安全施工第一责任人)对甲方安全管理人员提出的意见必须及时整改，否则，甲方有权责令乙方停止施工，由此造成的一切损失由乙方承担；若发生人身事故和不安全情况，乙方必须立即报告甲方单位负责人。8、乙方施工人员在施工过程中不得擅自进入非施工作业的甲方其它生产、办公场所，不得擅自操作任何电气设备。如有可能触及或接近高压设备的情况时，必须停止施工并主动与甲方联系，待采取安全措施并经甲方许可后方可继续施工。 9、本工程不得再行转包。三、施工期间甲方指派 同志负责联系、检查、督促乙方执行有

6、章： 日期： 年 月 日 日期： 年 月 日 临舍彩钢板房安装合同甲方：吴江市震泽万里彩钢板厂 乙方：杭州萧宏建设集团有限公司根据中华人民共和国合同法及有关法律法规精神，甲乙双方本着平等互利的原则，经友好协商一致同意签订本合同。一、 订购板房内容1、甲方向乙方订购的活动板房材质为彩钢岩棉板房（材质为岩棉A级防火，3.5mm基板）。板房包括双层办公、住宿楼7栋，单层厕所卫生间及盥洗室一栋，单层厨房、餐厅一栋，单层门卫室一栋。建筑安装总面积约 平方米，施工图纸详见附件一（临时设施平面布置图）。2、板房符合杭州市消防局印制杭州市建设工程施工工地消防安全管理标准及国家GB50720建设工程施工现场消防
7、安全技术规范要求，以及杭州市建委最新防火标准。二、 合同金额1、合同总价为 元（大写： 整），本合同金额包括附件一图中所有板房彩钢板岩棉板板材及板房基础，砼地坪，平改坡，阳台隔断板，所有房间隔断板、房间地板、所有房间布线（含电灯、电线、电话线、上网线、插座、开关），施工现场钢材板围档，厕所及盥洗室内所有设施，厨房及餐厅内所有隔断及水池砌筑等工作，所有房间的水管布置（含水管、龙头）以及相关的卫生清理工作等一切满足甲方使用条件的工作。2、项目详细报价表见附件二（合同预算书），本合同按照附件二内容进行验工及验收。3、本合同在预算书范围内的项目所有材料不受价格因素影响而调整。如甲方增加预算外施工内容，
8、经甲乙双方协商后作为补充合同另行结算。三、 付款方式1、在合同签订后，甲方支付乙方活动房造价的10预付款。2、在乙方所有钢结构及岩棉板材到场后，甲方支付活动房造价的40%，达到合同总价款的50%3、在所有工程完工乙方人员入住，消防局验收通通过后，乙方支付甲方活动房造价的45，达到合同总价款的95%，预留5质量保证金，保修期18个月满后予以支付。第四条 质量标准与保修期限乙方根据国家和地方的活动房质量标准向甲方交货，并能满足建筑结构荷载规范GBJ-87，办公室和宿舍150kg/m2,会议室200kg/m2的安全使用要求，活动房保修期限为18个月（包括房间排线）。期限为消防部门验收合格日起算。保修
9、期间维修活动房所发生的维修费用全部由乙方承担。乙方承诺在甲方通知后的24小时内到达服务现场解决问题，否则由此引起的损失由甲方在乙方质保金中扣除。第五条 安装地点及工期1、安装地点：甲方指定地点（ 下沙 ）2、安装布置以施工平面图为准。3、安装工期：从2012年3月1日至2012 年3月5日共计5个日历天。第六条 双方责任1、甲方应严格按照合同约定支付合同款项。2、乙方必须提供公司营业执照、板材出厂合格证、板材检验报告、板房施工方案、施工图纸等相关合同附件并配合甲方办理相关杭州轨道交通建设区域大临设施搭建审批表工作。3、乙方必须提供足够的人力保证按期完工并安全交付使用。4、付款时乙方必须提供国家
