32、XX电镀厂（电镀车间）建设项目职业病危害控制效果评价报告书 XXXXX号1 总论1.1项目背景温州工业以轻工产品为主，而电镀行业是轻工业很重要的配套产业。电镀是温州不可代替的基础性重要行业。苍南县XXXX电镀厂是一家股份制企业，新厂区坐落于苍南县金乡镇凉亭村XXXX号，老厂区（电镀车间）坐落于地址位于温州市苍南县金乡镇金XXXXXXX号，老厂区占地面积5800m2,整体分为东面厂区电镀车间和西面厂区电镀车间，中间已用实体墙隔开，两厂区均已独立设置污水处理设备、酸库，实际最大允许生产电镀容量为127428升。主要从事徽章、塑料标牌的电镀加工。年生产金属镀件4000万件、塑料镀件2000件。项目总
33、投资2700多万元，其中职业卫生防护投资70万元。目前该公司各职业卫生设施都已投入运行。镀种主要有镀酸铜、镀氰铜、镀铬、镀镍、枪黑、仿金等。根据浙环发201167号文件关于印发浙江省电镀行业污染整治方案的通知要求，公司于2012年初对现有的各类挂镀线进行技改，将原先的大部分手动电镀改为自动电镀线，2012年7月已进入技改后的运行生产，东面厂区电镀车间拥有挂镀铜镍铬环型自动线2条、前处理自动线1条，西面厂区电镀车间拥有挂镀铜镍铬环型自动线2条。东面厂区电镀车间内原手动线全部改造成了自动线已投入使用；西面厂区生产车间大楼正在建设，其中一楼电镀车间已经建设完毕，并已投入使用。本项目在可行性研究阶段，

42、项目试运行期间的作业现场职业卫生学调查，明确建设项目产生的职业病危害因素，并根据职业病危害因素检测数据，分析其危害程度及对劳动者健康的影响，评价职业病危害防护措施及其效果，对未达到职业病危害防护要求的系统或单元提出职业病控制措施的建议，从而从源头控制或消除职业病危害，防治职业病，保护劳动者健康；为职业卫生监管部门对建设项目职业病防护设施竣工验收提供科学依据；为建设单位职业病防治的日常管理提供依据。1.4评价范围根据企业的委托，本评价报告主要针对苍南县XXXX电镀厂电镀车间的生产过程进行评价，不包括该企业的污水处理工艺和新厂区（苍南县金乡镇凉亭村金灵路18号）。主要电镀生产系统包括电镀铜、镍、铬
43、、仿金、氰铜、枪黑、镀金工艺。根据本项目试生产期间产生职业病危害因素的情况，将该项目分为东面厂区电镀车间电镀工序和西面厂区电镀车间电镀工序共二个评价单元对生产工艺过程中的职业病危害因素进行识别，对是生产期间产生的职业病危害因素及其危害程度进行现场检测、实验室分析，对职业病危害防护设施防护效果以及职业卫生管理措施等进行分析与评价。本报告主要针对电镀车间试运行期间职业病危害防护设施及效果和职业卫生管理措施等进行评价。不包括该企业其他生产车间、生产工艺和本项目今后可能对工艺、设备等更改后的评价。1.5评价内容评价内容主要包括:总体布局及设备布局的合理性，建筑卫生学，职业病危害因素及分布、对劳动者健康
44、的影响程度，职业病危害防护设施及效果，辅助用室，个人使用的职业病防护用品，职业健康监护，职业卫生管理措施及落实情况等。1.6评价方法根据苍南县XXXX电镀厂设备更新技改项目职业病危害特点，通过职业卫生现场调查、职业病危害因素检测、职业健康检查等方法收集数据和资料，并结合职业病防护措施、个人职业病危害防护水平和定量分级，对试运行期间作业人员的职业病危害因素接触水平及职业健康影响进行评价，主要采取以下几种评价方法：1.6.1职业卫生调查1.6.1.1企业基本情况调查主要内容包括工程概况、试运行情况、总体布局、生产工艺、主要生产设备及布局、使用的原料及辅料、副产品及企业职工人数，各个岗位的设置情况、
45、工作制度等。1.6.1.2职业卫生情况调查在分析本项目基本资料的基础上，对企业的职业卫生情况进行调查，内容主要包括职业病危害因素以及时空分布、各岗位接触职业病危害因素的情况、职业病防护设施设置及运行、个人使用的职业病防护用品的配备及使用、应急救援措施的实施、建筑卫生学、辅助用室的设置、职业健康监护、职业卫生管理的落实情况等。1.6.1.3工作场所调查调查内容包括：总平面布置、设备布局、生产工艺流程、生产过程中的原辅料、给排水、绿化、采光照明、采暖通风及岗位运行规程、运行管理制度。1.6.1.4 工作日写实写实对象和人数的确定原则：选择各评价单元中主要生产岗位有代表性员工为写实对象，对工作随意性大的岗位，全员写实。写实动作分类原则：劳动强度相同或相似的动作分为同类；同名工作尽可能分在同一类；动作内容相近，作业环境明显不同的动作分类统计；动作时间少，或偶尔出现的动作，可归类到与其相近的动作。本次评价项目中以固定岗位为主。通过工作日写实，了解各岗位的工
