1、上海华虹宏力半导体制造有限公司华虹宏力三厂增资扩产项目洁净室及一般机电系统项目带压开孔施工方案编制：审核： 批准：目 录一、工程概况二、编制依据及执行标准 三、现场管道带压开孔施工技术方案介绍四、现场施工方案五、施工应急预案六、应急预案启动模式七、主要工程量八、施工总部署 九、质量保证体系、措施十、文明施工一、工程概况1、工程概况：华虹宏力工厂FAB栋四层新增电气室外部增加两台FCU，FCU冷冻水管路需要与原有管线连接。由于原有管线无法停产，与业主沟通后本次新旧管线连头采用带压开孔作业，既不影响设备的正常运行，又能保证安全、高效、环保的完成新旧管线的连接工作。2、工作内容：工艺冷冻水主管道增加
2、输出接口。二、编制依据及执行标准：（1） 施工蓝图及现场交底；（2）钢制管道封堵技术规程 第2部分:挡板-囊式封堵SY/T6150.2-2003；（3） 有关国家、行业标准和规范，相关技术文件等；三、现场管道带压开孔施工技术方案介绍1、现场施工工艺流程施工准备设备入场确定带压开孔点开挖操作坑（如果需要） 焊接法兰短节 安装闸板阀与开孔钻机-整体试压 开孔作业关闭闸板阀拆除钻机 安新管线连头清理现场施工完毕2、管道带压开孔工艺介绍管道带压开孔是指在密闭状态下，以机械切削方式在运行管道上加工出圆形孔的一种作业技术。当在役管线需要加装支管时，可采用管道带压开孔技术完成，既不影响管线的正常输送，又能保
3、证安全、高效、环保的完成新旧管线的连接工作。管道开孔目的在于管道在不停产带压的情况下焊接法兰短节后，按要求安装闸板阀、开孔机等设备，在管道上进行开孔，开孔为管道的等径孔，也就是开孔直径与管道内径一致。 四、现场施工方案1、施工前的准备（1）根据施工工艺参数，编写详细的施工方案。（2）施工设备进入现场前，对每一个设备的性能进行检查。确保进入现场施工的设备完好齐全。（3）对进入现场施工的人员进行安全教育培训。（4）一些易损和消耗材料准备充分，防止发生施工怠工现象；对此可以将消耗材料先预计使用量，在备有库存；施工机具方面可以准备备用件。2、法兰短节焊接前准备（1） 法兰短节焊接前首先确认法兰短节材质
4、应与老管线相符。（2） 开孔位置要选择腐蚀点少的地方作为焊接点。（3） 若不清楚内壁腐蚀情况必须用测厚仪进行测量再进行焊接。（4） 根据钢制管道封堵技术规程 第2部分:挡板-囊式封堵SY/T6150.2-2003标准中关于管道允许带压施焊的压力计算公式，如下计算。 P=2s（T-C）DF P=管道允许带压施焊的压力，单位为Mpa s=管材的最小屈服极限，单位为Mpa T=焊接处管道实际壁厚，单位为mm C=因焊接引起的壁厚修正值，单位为mm，一般取2.4mm D=管道外径，单位为mm F=安全系数，（原油、成品油管道取0.6，天然气、煤气管道取0.5）3、法兰短节焊接方案（1） 选择好开孔点后
5、进行法兰短节的焊接，法兰短节焊接要严格遵守钢制管道封堵技术规程 第2部分:挡板-囊式封堵SY/T6150.2-2003中规定的焊接顺序进行焊接。（2） 法兰短节的焊接严禁使用气焊。（3） 法兰短节的焊接贴进母管位置焊接采用3.2mm焊条小电流焊接。 （4） 法兰短节焊接时电焊机二次线应放置在法兰短节的上面并接触牢固。（5） 此项环节为施工重点环节，施工为了保证安全，除以上措施必须做到外，要保证：施工现场要有专人监护。4、 管线开孔方案（1） 法兰短节焊接完毕后要组装闸板阀，在安装前要启闭开关两次再装到法兰短节上面去。防止闸板阀装上去打不开，开完孔闸板阀关闭不上。（2） 管线开孔前要检查开孔刀在
6、连箱里的位置是否有偏心现象，若有则卸下刀具，先用仪器测量开孔连箱是否中心，然后测量刀具中心位置，再把刀具搬紧。（3） 开孔时要注意钻机的转数，应控制在开孔刀22m/min以内，液压站运行压力应控制在2.5Mpa以下，额定排量在40L/min左右。（4） 如果开孔机再开孔过程中出现刀具卡住现象，一般有两种可能性，第一就是液压站设定压力太小或者排量太小，这时停机重新调整液压站设定数据就行；第二就是刀具切削正常卡住，这时不能慌乱，首先要把液压站停机，把钻机挡位换到空位，用摇把手动逆时针盘动钻机减速箱大轮，然后挂入进给挡继续切削。（5） 按照钻机转数及开孔大小计算开孔时间（钻机额定进给量M=0.1mm
7、/转），如下：开孔时间T=H（转数/minM） （6） 开孔完毕后，将开孔刀及马鞍块提入钻机连箱内，关闭闸板阀后将连箱内的介质放干净，拆下开孔机开孔完毕。五、施工应急预案管道带压开孔施工，本身就是一种高危险的作业，特别是油气管道，更是慎之又慎，本次所要施工的管道介质是水，所以每一步的操作作业都存在一定的风险，主要步骤有：1、管道带压焊接；2、管道带压开孔。有关这些主要的施工步骤有可能发生的危险及相关的应急预案在下面一一说明。1、 法兰短节的带压焊接危险因素：焊接时击穿，导致里面的介质泄漏。预防措施：1）焊接点必须做测厚，计算允许带压施焊的压力。2）焊接时采用小电流缓慢焊接。禁止采用气焊。3）焊
8、接及操作人员的个人PPE必须合适合格，焊工穿戴好专用工作服及专用安全帽。应急预案：如果焊接时出现电流击穿，首先焊接人员从现有逃生通道13号楼梯口逃生，同时有现场监护人员通知业主，采用导水带接入附近排水口排水。若是操作人员受伤，及时用电话呼叫急救电话：120.2、 管道带压开孔危险因素：1）开孔作业时介质泄露；2)开孔时刀具被卡住，导致开孔机拆不下来。预防措施：1）开孔机安装完毕后，必须先对法兰短节，闸板阀、开孔机做整体气密性试验，有开孔机放空阀处注入氮气，检查各个连接面和焊口没有泄露时方可进行开孔。2）调整开孔机系统压力，防止刀具因为扭矩不够而产生卡刀。v 应急预案：刀具如果被卡住，而且通过调
9、整系统压力增大扭矩的办法，解决不了。应当在开孔点，后方重新焊接法兰短节，进行开孔作业。 六、应急预案启动模式1、施工组织人员设计 管道主管：李贯章设备负责人：郑严华安全负责人：张少峰施工负责人：张红兵技术负责人：吴尚秋施工人员管工：黎士祥焊工：李宝卫设备操作工：郑严华安全小组运行模式：1、 加强员工安全意识，每一个细节不能疏忽，大家都有责任感。2、 员工进入现场劳保穿戴齐全。3、 安全监督员要负责周围环境，不能动用明火，施工30米范围内除施工人员外不能进入，要有隔离带。4、 项目部建立应急小组，项目经理为小组直接领导者，紧急情况发现时，启动应急小组。5、管线施工时指定专人监护，发现情况及时向上

12、学合理的施工进度计划，作为工程施工的指导性文件，并根据工程的实际进度情况及时调整，以确保工程按期完成。5、制度保证建立施工进度奖惩制度，调动和激励员工积极性，在保质保量的前提下，确保进度计划的执行。6、后勤保证本项目配备专职的综合管理员负责后勤保障工作，为施工人员做好后勤保障工作，以确保工程的顺利进行。9.2保证质量的技术措施：1、质量方针和质量目标1.1 质量方针：紧密结合、安全畅通、诚信为本、技术领先1.2 质量目标：1.2.1工程竣工验收一次合格率100%；1.2.2合同履约率100%；1.2.3 顾客满意率100%； 1.2.4 每年至少完成2项技术改进。2、组织机构及主要人员职责2.
13、1. 人员职责 工程师协助项目经理监督/管理项目工程的各项技术活动。根据项目需要，组织有关部门及人员研究制定技术方案，解决技术难题。协助项目经理负责项目技术工作。为项目决策提供技术咨询、建议。项目质量代表，全面负责项目的质量保证和质量控制工作。质保工程师负责施工现场的技术和质量管理工作，随时处理施工中出现的技术和质量问题。熟悉和掌握本工程的设计和规范要求，并贯彻执行。负责向施工人员进行技术交底，使所有施工人员都能掌握本专业范围内的质量标准和技术要求。9.3质量保证措施9.3.1 按照施工规范，加强现场管理工作，使质保体系在施工中得到有效地运行。9.3.2 加强现场施工质量检验工作。施工部要坚持
14、现场质量巡回检查制做到积极预防和严格检验相结合，以自检为主的原则。9.3.3 尊重甲方、服从监理，及时做好各道工序的报验和报批等工作。9. 3.4 认真贯彻落实各种质量管理制度；制定作业指导书；建立设备、材料交接、验收制度；执行设计变更审批制度；现场设备管理制度；生产调度制度；计划报批制度；质量检查检测制度。9.3.5实行技术质量责任制，项目经理对工程质量全面负责，质量管理实行定点、定位划片负责制，各负其责，做到个人的工作有据可查。1） 严格“三按、三检、三工序”制度。每道工序完成后必须进行自检和互检，合格的方可移交下道工序。2） 制定各级各层质量工作岗位责任制，做到纵向到底，横向到边，克服重
15、进度、轻质量的思想，加强工序间的自查互查。质量检查员应组织操作人员对质量问题进行分析，找出问题原因及薄弱环节，制定对策。3） 各施工部要组织好每周至少一次的质量分析会，做到现场施工情况明确，分析质量问题准确，措施得力。4） 对生产过程中发生的质量问题，要做到“三不放过”原因不清不放过，责任不清不放过，措施不落实不放过，杜绝质量隐患，防止重复质量问题的发生。5） 施工部要加强质量、安全、技术员对工程质量、安全的直接管理及监督。项目部的质量管理人员、技术负责人、项目总协调要定期或不定期地进行全线检查和抽查，分期检查审核，配合接受现场监理的检查，使工程施工处于全过程的受控状态。实行公司的质量否决权贯
16、彻公司质量奖惩制度。6） 对技术复杂，施工难度大的问题，要由工程技术人员和技术部专家共同研究，找出合理的施工方案，确保工程质量。7） 严格按工程技术文件及施工验收规范要求施工，技术质量人员必须对项目严格监控核查。8）施工过程的变更必须取得设计人员、甲方的认可后，并及时办好有关的变更手续，方可进行施工。9）开展质量意识教育和技术培训，提高各级技术人员和施工人员的质量意识和技术业务水平。重要岗位操作人员必须持证上岗，加强对特殊工种的岗前培训。对管理者、经营者加强全面质量管理的教育和提高。10）强化施工质量的监督控制，加强施工质量的中间检验，上道工序不合格，不能进入下道工序施工，对重要工序如焊接、开
17、孔等，未经现场工程师人员认可，不能进入下道工序施工。既要尊重业主，又要通过本次施工提高队伍素质，提高企业质量第一服务宗旨。11）对制定的质量目标和质量通病，开展广泛的质量攻关活动，解决通病，克服难关，在较短时间内，进入良好的施工状态，从而达到施工质量目标。12) 各施工部要接受阶段性质量大检查，对查出的质量问题制订整改措施，一丝不苟、严肃认真对待检查提出和发现的各种大小问题，最终使施工质量达到标准要求，交给甲方一个满意的工程。十、文明施工1、文明施工的目标：无因工伤亡和重大机械设备事故无火灾、爆炸事故2、文明施工的措施：加强职工职业道德教育，提高文明施工思想意识，遵守职业道德，遵守公司规章制度
