4、22.520万吨级码头B1型117.553125D型717.5517.9518直立式引堤D1型117.5517.9520.5S型522.816.318引堤及预留顺岸码头ST型115.113.9182.工程参建单位建设单位烟台港西港区发展有限公司设计单位中交第一航务工程勘察设计院有限公司监理单位山东港通工程管理咨询有限公司施工单位总包：中交第一航务工程局有限公司分包：烟台中交航务工程有限公司3. 工程特点（1）沉箱单个重量达到7000吨，地面承载力要求高。要验算使用预制场是否能达到要求。（2）沉箱高度最高达到27.5米，施工分层多，起吊和垂直运输难度大。施工效率较低。（3）本工程工期紧张，按照目
5、前的工作量，平均每月要预制 个，最集中时砼的施工方两量能达到 立方。其中7000吨沉箱的出运、安装为国内之最，施工工艺复杂，船机设备要求高。如不合理安排工期势必影响到沉箱的预制、出运及安装。二、沉箱预制分项工程的监理控制目标类 别内 容质量目标沉箱预制质量等级必须达到合格，确保沉箱能顺利出运、安装就位。进度目标沉箱预制总工期14个月。投资目标施工合同额 万元，属于固定总价合同或？安全目标行使安全监理责任，力求无较大安全生产责任事故，无重大人员伤亡事故发生环保目标行使环保监理责任，力争不发生环保事故和纠纷，确保工程施工不对环境造成永久影响。组织协调协助业主做好现场的组织协调工作。三、 监理工作的
6、组织策划（一）收集准备监理依据性工作文件1从业主处领取并熟悉以下资料烟台港西港区油品码头水工工程施工合同；烟台港西港区油品码头工程施工图纸；烟台港西港发展有限公司总体施工安排；地质报告；坐标控制点；地下管网交底资料（如有需要）等。仔细研读施工合同，掌握合同内容，特别是合同价款、工程数量、工程价款调整方式等内容。2.识别并领取本专业工程需要的技术图书规范识别本专业工程所需要的技术图书规范标准，如果项目监理部没有，向公司申领。本工程需要的技术资料如下：（1）水运工程混凝土施工规范JTJ268-96（2）水运工程混凝土质量控制标准（JTJ269-96）（3）水运工程混凝土试验规程JTJ270-98（

9、同，项目监理部的监理规划，掌握合同内容，依据监理规划编制监理细则。4.施工单位报送的施工组织设计以及专项施工方案（根据工程需要） （二）专业监理工作的预控与策划工作1调查掌握现场施工环境和工作条件（1）收集当地天气情况和水文潮汐情况，调查陆上预制的施工条件和沉箱出运的条件，统计分析本工程的有效作业天数。（2）掌握当地水泥、钢材、砂、石子等建筑材料质量和市场供应情况，施工用水的水源情况。（3）落实现场道路、水、电、通讯信号等情况。（4）熟悉预制厂的整体结构、位置及台座数量；了解预制厂便道的布置情况；根据沉箱设计图纸的尺寸及重量情况核实台座是否满足本工程的需要（核实过程则需要施工单位提供预制厂施工
10、图纸及施工资料）；进行预制厂周边环境调查，预测预制厂施工是否对周边村民生活以及环境造成影响，如有影响则需采取相应的控制措施。2图纸审查熟悉图纸，全面了解设计意图；审查图纸是否表达了设计意图；根据工程特点及个人工作经验提出合理化建议；提出书面审查意见（可在监理日记上记录），参加图纸会审。图纸主要从以下几个方面审查（根据工程情况进行删减调整）：(1) 图纸是否经设计单位正式签署。 (2) 测量、地质勘探资料是否齐全。 (3) 设计图纸与说明是否齐全，有无分期供图的时间表。 (4) 设计地震烈度是否符合当地要求。 (5) 几个设计单位共同设计的图纸相互间有无矛盾；专业图纸之间、平立剖面图之间有无矛盾
11、；标注有无遗漏。 (6) 总平面与施工图的几何尺寸、平面位置、标高等是否一致。 (7) 防火、消防设计情况，是否满足要求。 (8) 建筑结构与各专业图纸本身是否有差错及矛盾；结构图与建筑图的平面尺寸及标高是否一致；建筑图与结构图的表示方法是否清楚；是否符合制图标准；预埋件是否表示清楚；有无钢筋明细表；钢筋的构造要求在图中是否表示清楚。 (9)材料来源有无保证，能否代换； (10)图中所要求的条件能否满足；新材料、新技术的应用有无问题。 (11) 地基处理方法是否合理，建筑与结构构造是否存在不能施工、不便于施工的技术问题，或容易导致质量、安全、工程费用增加等方面的问题。(12) 工艺管道、电气线

16、解施工工序，了解本专业工程的强制性规范条文并汇总，在工程监理过程中遵照执行。本专业工程的强制性规范条文共11条，汇总见附件4。6质量通病的防治针对本工程的特点，提前了解分析本工程可能发生的质量通病，制订相关的预控措施，防止质量通病的发生。本工程的质量通病包括工程实体质量通病.施工工艺质量通病和施工管理质量通病三个方面。工程实体质量通病包括沉箱预制砼施工缝处理缺陷、沉箱混凝土表面缺陷、钢筋砼保护层偏差过大缺陷。施工工艺质量通病包括保护层垫块制作工艺落后、混凝土施工缝处理不规范、混凝土分层施工缝漏浆及错台大、混凝土修补质量欠佳。施工管理质量通病包括质量责任不明确、落实不到位，隐蔽工程验收不符合规定

18、详见附件6。（三）召开专业工程监理交底会议，明确监理工作程序和要求。专业工程开工前，召集第一次工地会议，会议参加人员有施工单位、建设单位代表。提出工作要求，明确工作程序。监理人员需要提前准备第一次工地例会通知和监理交底。四、监理工作的控制要点及控制措施（一）检查施工单位的施工准备情况1审查承包商资格沉箱预制由烟台港西港区油品码头工程总承包单位按照施工合同约定进行了分包，审查分包合同及分包商的资质，包括营业执照、企业资质、安全生产许可证。2审查承包商报审的施工组织设计督促承包商按照合同要求及时编制、提交施工方案，并进行审查。施工方案不合理或无法满足合同的质量、工期及安全环保要求的内容，要求修改和
19、完善。从以下方面进行施工组织设计进行审查：审查的主要内容应达到的标准气象和施工环境条件的分析、调查全面、准确，施工工艺、劳动力、材料供应、施工设备选型、计划安排充分考虑当地自然条件的影响。施工总平面布置符合现场条件，有利于生产组织和提高生产效率。预制场排水系统通畅，避免受雨水及混凝土养护用水浸泡预制场地基导致预制场地地基沉陷。沉箱出运系统满足沉箱出运的正常需要。现场临建符合施工现场临时建筑物技术规范JGJ/T188-2009规定。对临时用电方案进行审查。质量岗位责任制及质量体系制度合理健全，技术管理制度有利于工程质量，施工技术交底制度能够得到落实。检查承包商材料管理人员和材料管理制度有利于工程
20、质量和文明施工。关键工序施工方法满足设计要求，安全、可靠。对脚手架、模板的设计计算参数进行验算。总进度计划满足合同工期要求，设备、材料、人员的配置满足工程进度的要求。否符合法律法规和规范标准符合法律法规和规范标准、特别是工程建设强制性标准的要求。杜绝因施工方案存在不足，导致群死群伤安全事故发生。签字盖章程序方面文件必须先经编制人签字、施工单位技术负责人审查签认，并加盖施工单位公章。3审查承包商主要管理人员和特种作业人员资格（1）审查承包商主要管理人员资格建立控制台帐，对承包商主要管理人员实行动态管理，在工程实施过程中及时跟踪人员证书的有效期。台帐格式见下表：管理人员岗 位投标人员进场人员进场人
