This revised edition supersedes BS 5882 : 1990, which is withdrawn, and brings the specification into alignment with BS EN ISO

10、9001 : 1994. These changes reflect experience in the use of the specification and updates in the light of modern practice. It attempts to establish compatibility of quality assurance principles between existing documents such as annex B of USA Code of Federal Regulations 10 CFR 50 1, American Nation

11、al Standards Institute (ANSI/ASME) NQA 1 2 and the International Atomic Energy Agency Code of Practice 50-C-QA 3 4. It sets out principles for the establishment and implementation by the owner and the owners agents of a total quality assurance programme for a complete nuclear installation, and for e

12、ach of the constituent areas of activity relevant to an operating installation. It covers all the stages of activity such as design, procurement, manufacture, construction, commissioning, operation, modification and decommissioning of structures, systems and components of nuclear installations. The

13、proper establishment and implementation of a total quality assurance programme from the design through to decommissioning of a nuclear installation is necessary to provide adequate assurance of quality to meet statutory requirements imposed by regulatory authorities. In order to achieve this objecti

14、ve it is important to recognize that within the total quality assurance programme adopted, the basic responsibility for quality achievement in the performance of a particular task (for example, in design, in manufacturing, in commissioning, in operation) belongs with the individual or individuals as

15、signed the task, as well as with those seeking to ensure by means of verification that it has been achieved. It is emphasized that nuclear installations will not be designed, manufactured, constructed and operated successfully solely as a consequence of the preparation of a quality assurance program

16、me since it is the application of the quality assurance programme that is important. Quality assurance is an essential aspect of good management. Good management contributes to the achievement of quality through thorough analysis of the task to be performed, the identification of the skills required

17、, the selection and training of appropriate personnel, the use of appropriate equipment, the creation of a satisfactory environment in which activity can be performed and a recognition of the responsibility of the individual who is to perform the task. A quality assurance programme provides for a di

18、sciplined approach to all activities affecting quality including, where appropriate, verification that each task has been satisfactorily performed, and production of documentary evidence to demonstrate that the required quality has been achieved. 20、(c) BSI BSI 19961 BS 5882 : 1996 Specification 1 Scope This British Standard specifies principles for the establishment and implementation of quality assurance programmes: a) during all stages of: design; procurement; manufacture; construction; commissioning; operation; modification, and; decommissi

21、oning; b) of structures, systems and components of nuclear installations including: power plants; fuel fabrication plants; fuel processing plants; spent fuel storage facilities; fuel re-processing plants; waste treatment; packaging; handling; storage; disposal facilities, and; installations involved

22、 in the production of radioactive isotopes. These principles apply to activities affecting the quality of items, including: designing; purchasing; manufacturing; handling; shipping; storage; cleaning; erecting; installing; testing; commissioning; operating; inspecting; repairing; modifying and, even

23、tually; decommissioning. NOTE. The manner in which the principles described in this standard will be implemented in the different organizations involved in the various stages of a specific project, contract or installation will depend on regulatory and contractual requirements, the form of managemen

24、t applied and the nature and scope of the work to be performed by the different organizations. 2 Responsibility It is the responsibility of the owner of the nuclear installation to ensure that a total quality assurance programme for the installation is established and executed in a manner consistent

25、 with the provisions of this standard. The owner is permitted to delegate to other organizations or to specialists the work of establishing and executing a quality assurance programme, or any part thereof, but shall retain responsibility for overall programme effectiveness. In no way shall a total p

26、rogramme operate to diminish the responsibility of an individual organization or participant for the quality of items or services furnished or for the activities executed. 3 References 3.1 Normative references This British Standard incorporates, by reference, provisions from specific editions of oth

27、er publications. These normative references are cited at the appropriate points in the text and the publications are listed on the inside back cover. Subsequent amendments to, or revisions of, any of these publications apply to this British Standard only when incorporated in it by updating or revisi

28、on. 3.2 Informative references This British Standard refers to other publications that provide information or guidance. 29、For the purposes of this British Standard the definitions given in BS EN ISO 8402 : 1995 and BS 4778 : Part 2 : 1991 apply as follows, except that in this standard the term item replaces entity: 4.1 code A standard which satisfies a regulatory body. 4.2 commissioning The initial testing and start-up

30、 process by which a nuclear installation is put into operation. 4.3 decommissioning The permanent retirement from service of a nuclear installation or part thereof and the work which follows in bringing it to a definitive condition. 4.4 documentation Any recorded or pictorial information describing,

31、 defining, specifying, reporting or certifying activities, requirements, procedures or results. 4.5 item An all-inclusive term covering structures, systems, components, parts or materials. 32、I 1996 BS 5882 : 1996 4.6 non-conformance A deficiency in characteristics, documentation or process implementation which renders the quality of an item indeterminate or outside that required by the specification. 4.7 operation All activities performed to achieve, in a safe manner, the purpose for wh

33、ich the installation was constructed including maintenance, refuelling, in-service inspection and other associated activities. 4.8 owner The organization (industrial or governmental) which has overall responsibility for the nuclear installation. 4.9 quality plan Document setting out the specific qua

34、lity practices, resources and sequence of activities relevant to a particular product. NOTE. A quality plan may reference the quality manual or quality assurance programme. 4.10 quality assurance programme A description of the overall management and procedures to provide quality assurance for the ex

35、ecution of a specific contract or project for a nuclear installation, at any stage in its life cycle from design to decommissioning. 4.11 review An independent appraisal undertaken by an individual or group competent in the area being considered. 4.12 safety documentation Documentation provided to d

36、emonstrate that the nuclear installation conforms (or will conform) to specified criteria for nuclear and radiological safety. 4.13 services Functional expertise or specialist activity provided in support of the main activity. 4.14 supplier evaluation An appraisal to determine whether or not a suppl

37、ier is capable of producing an item or service in accordance with a specified requirement, and providing objective evidence thereof. 4.15 testing The determination or verification of the capability of an item to meet specified requirements by subjecting the item to a set of physical, chemical, envir

38、onmental or operating conditions. 5 Quality assurance programmes 5.1 General 5.1.1 Quality assurance programmes shall conform to the applicable clauses of this standard and shall be established at the earliest practicable time consistent with the schedules for implementing the activities associated

39、with particular stages of a contract, project or installation. The programme shall identify quality policy, which shall be defined by management having executive responsibility for quality. This policy shall include broad objectives and the organizations commitment to quality, and its implementation

Programmes shall identify the procedures for implementation of the various activities, and shall include the methods of assuring that items and services are supplied in accordance with the specified requirements and the installation is operated in accordance with specified procedures. 5.1.2 Progra

41、mmes shall define the structure, responsibility, levels of authority and the internal and external interface arrangements of the personnel and organizations involved. NOTE. The total quality assurance programme for a complete installation may comprise a number of individual quality assurance program

42、mes prepared by the various participants to cover activities associated with particular stages of a contract, project or installation. 5.1.3 Programmes shall identify the items, activities and services to which this standard applies. NOTE. Since items, activities and services will differ in regard t

43、o the relative importance of safety, reliability, performance and environmental considerations, it is possible to use various quality assurance standards and methods of control and verification to assure the required quality. Regardless of the methods used, the programmes should provide for the assu

44、rance of quality consistent with the installations safety documentation, applicable codes, standards and other requirements. As a guideline, some factors to be considered in determining the appropriate assurance are as follows: a) the consequence of malfunction or failure of the item; b) the design

45、and manufacturing complexity of the item or its novel features; c) the need for controls over processes and equipment; d) the degree to which functional compliance can be demonstrated by inspection or test; e) the quality history and degree of standardization of the item; f) the difficulty of repair

46、, replacement or accessibility for in-service inspection. 5.2 Quality planning The methods used for quality planning shall be defined. Quality planning shall be consistent with all other requirements of the quality programme and shall be documented in a format to suit the organizations method of ope

47、ration. 5.3 Documentation and management aspects of a quality assurance programme 5.3.1 The programme shall be implemented by quality plans, procedures and instructions. The programme shall identify arrangements which assure compliance with other standards, engineering codes and practices. Documenta

48、tion relevant to the appropriate elements and activities shall be identified in the programme. 5.3.2 The programme shall identify arrangements for training and qualification of personnel performing activities affecting quality to ensure that suitable proficiency is achieved and maintained. NOTE. Controlled conditions include the use of appropriate equipment, suitable environmental conditions and assurance that prerequisites for the given activity have been satisfied.
