1、2015 家庭医生签约服务工作总结家庭医生签约式服务工作总结2015 年度为进一步推进社区卫生服务能力建设深入贯彻落实社区卫生服务中心家庭医生式健康管理签约服务工作方案我中心责任医师团队开展了一系列的宣传和签约工作现将本团队家庭医生签约式服务工作总结如下:一、 高度重视，积极部署及时组织团队成员学习工作方案精神紧密围绕家庭医生式服务工作的基本原则、服务内容、服务对象、工作持续性等内容进行了充分研讨统一了思想提高了认识。为家庭医生式服务工作的稳步推进奠定了组织基础。二、广泛宣传，深入动员为保障家庭医生式服务工作的深入人心我中心统一印制了以宣传家庭医生服务理念和服务内容、服务形式为主题的“致社区居
2、民一封信”并分发给本辖区各居委会同时印制家庭医生联系卡、家庭医生式服务协议书等制作了宣传横幅、宣传栏。有效营造了家喻户晓的宣传氛围为家庭医生式服务工作的顺利推进奠定了舆论基础。三、调查需求，个性服务the first level to consider when positioning security, roughly the same distance between the point and level to put the stand, can't be moved, dislocation, and so on. Pay attention to the proper us

7、e在签约服务的居民中调查了居民希望得到的医疗卫生服务。包括健康教育与健康促进、慢性病管理、孕产妇保健管理、残疾人康复、婴幼儿保健及其它服务,出诊、送药上门、家庭护理等, 。服务人群底数的清晰和需求人员数量的掌握为家庭医生式服务的扎实推进奠定了信息基础。四、明确对象，按需管理进一步明确辖区人口分布保证所有居民均能得到社区卫生服务体系覆盖。我团队组成由“全科医生、社区护士、防保人员”为核心的“片儿医”团队提供家庭医生式服务。明确所管辖的社区分布并在社区设立“片儿医”公示牌公示团队服务人员、服务范围、服务时间、服务内容、联系方式等信息。对愿意接受家庭医生式服务的居民按照健康状况和健康需求情况划分为四
8、类: 第一类为健康普通人群第二类为需关注的人群, 第三类为慢性病人群第四类为高危或合并严重并发症病人、残疾人、特殊病人。根据居民所处的类别明确服务内容及标准提供针对性的签约服务并保证服务的质量和可持续性。五、优先签约，有效服务优先与辖区内的慢性病人、空巢老人、残疾人、妇幼保健对象等愿意接受家庭医生式服务的重点人群签订家庭医the first level to consider whenpositioning security, roughly the same distance between the point andlevel to put the stand, can't be

(3) Note生式服务协议书并根据协议书内容提供建立健康档案、电话咨询、入户访视、健康教育等针对性服务。目前已签约的社区居民家庭50 户签约人数 59人。六、取得的初步成效家庭医生式服务模式实现了现有医务人员对社区居民健康管理的全覆盖从坐等患者上门变为深入社区为居民提供服务社区健康管理能力得到切实加强。提高了社区居民对医疗健康服务和健康知识的知晓率健康教育、预防保健、康复、计划生育等服务得到落实居民健康意识不断增强。在一定程度上缓解了困难群体“看病难、看病贵”的问题推进了基本公共卫生服务事业的发展。医师团队二一五年
