34、法、生产安全事故应急救援预案管理办法等法律法规；生产经营单位安全生产事故应急预案编制导则等技术标准及规范；本石场主要相关规章制度，以及田东县安监局生产安全事故应急预案来编制本预案。具体编制依据见附件1，石场生产安全事故综合应急预案编制依据一览表1.3 适用范围 本预案适用于本石场生产经营活动中发生的各类生产安全事故的应急救援工作。1.4 应急预案体系1、本石场的预案构成体系为采石场综合预案，专项预案，现场处置方案。结合本石场实际情况，现场处置方案作为综合预案的附件附于综合预案之后。2、本预案向上与田东县安监局生产安全事故应急预案相衔接，向下与专项预案及现场处置方案相衔接。1.5 工作原则 （1
35、）以人为本，安全第一。把保障职工的生命安全和身体健康、最大程度地预防和减少安全生产事故造成的人员伤亡作为首要任务。切实加强应急救援人员的安全防护。 （2）预防为主，平战结合。贯彻落实安全第一、预防为主、综合治理的方针，坚持事故应急与预防工作相结合。做好预防、预测、预警和预报工作，做好常态下的风险评估、物资储备、队伍建设、完善装备、预案演练等工作。 2生产经营单位的危险性分析2.1生产经营单位概况1、本石场位于田东县平马镇游昌村龙昌屯龙了山一带，距离市区约6公里左右，矿区面积0.07km2。开采标高+400+120m,年产量10万吨/年，从业人员20人。2、石场附近周边均为山坡，无农田，旱地分布
36、，矿山自然环境优越。矿区周边300m范围内无铁路、高压线、通讯电缆等经过，无其他矿权设置和非法采矿存在。石场建立了较为完善的安全责任制度以及安全生产管理制度，矿长（法人）对安全生产负总责，另配有专职安全管理人员2名。3、本石场设矿长、安全管理人员、财务部、生产安全科、办公室3个职能部门，生产安全科下设炸药库以及矿区2个小组。同时本石场另设一个负责整个石场设备维修的机修组，本石场组织结构详见附件4，石场织结构图。2.2危险源与风险分析（1）坍塌事故 石场在凿岩、爆破、装卸及处理松坊过程中可因采场边坡不稳而引起坍塌事故，特别是在爆破后和雨后，更容易发生。坍塌事故因塌落物自重大，作用范围广，往往伤亡
37、人员多，后果严重，多为重大或特大人身伤亡事故。（2）电气事故 在生产过程中，因电工人员违章检修或操作工操作不当可能发生触电、电弧灼伤、电气短路、起火等事故。电器设备及线路老化失修造成的电器设备绝缘破坏、漏电、电气短路事故；（3）爆破事故 石场爆破事故主要来自爆破作业过程中或由于矿岩节理裂隙发育以及爆破震动波、冲击波、飞石的影响，引起对作业人员造成潜在的危险。（4）车辆伤害事故 该石场生产采用汽车运输，还有装载机和其它辅助性车辆，存在车辆危害风险。（5）机械伤害 机械伤害是矿山生产最常见的伤害之一，采石场各种机械设备都可能造成机械伤害。这些机械设备包括：穿孔机械、装载机械、运输机械以及空压机、电
38、机等转动设备等。机械伤害主要包括挤压、夹击、碰撞、卷入、绞、碾、割、刺、剪等形式的伤害。（6）高处坠落与物体打击危害 石场在凿岩、搬运矿石；设备运行及维修过程极易引发物体打击事故，在台阶边缘作业或行走、前期开拓阶段、边坡作业过程中存在高处坠落的危险。（7）高温危害 本采石场为地表露天开采，石场气候属于亚热带季风气候，四季分明，春季雨日较多，夏季高温、高湿，在夏季高温环境中作业，由于不良气象因素的综合作用，可使体温调节系统、水盐代谢系统、循环系统、消化系统和神经系统产生生理机能的改变和障碍，工人在高气温与强热辐射的环境中操作，容易受到高气温与强热辐射的环境影响，如防护不当，可发生中暑，损害工人健
39、康，甚至造成死亡。（8）该石场还有临时存放油料和爆破器材的场所及压力容器空压机，还可能存在有火害和压力容器爆炸危害。3组织机构及职责 3.1常态下应急组织机构3.1.1应急管理委员会本石场设安全生产事故应急管理委员会（以下简称应急委），由应急委领导整个采石场的生产安全事故应急管理工作。应急委主任由石场主要负责人担任，副主任由安全主管担任。成员由生产安全科、办公室等部门负责人组成。应急委的工作职责主要是：1、领导石场安全生产应急管理工作。2、组织制定和实施本石场安全生产应急救援体系建设规划，批准年度安全生产应急管理工作计划，督促检查各项工作的落实情况，协调解决安全生产应急管理工作中的重大问题。3
40、、决定安全生产应急管理的重大事项和应急救援行动的重大决策。4、审定安全生产应急救援相关工作经费。5、审定本石场安全生产事故综合应急预案和各类现场处置方案。3.1.2应急管理办事机构应急委下设立生产安全事故应急办公室（以下简称应急办），应急办设在本石场办公室。应急办主任由本石场安全主管长担任，应急办副主任由本石场生产主管担任，成员由生产安全科、办公室等有关部门人员组成。如有需要，由办公室提出申请，经应急办主任批准，可从其他部门相关抽调人员。应急办工作职责：1、在应急委的领导下承担安全生产事故应急救援准备日常管理工作。负责拟定本石场安全生产应急救援体系建设规划、年度安全生产应急管理工作计划，提出本
41、石场安全生产事故应急救援体系建设项目建议。2、负责上级有关安全生产应急救援工作方面指示精神的贯彻落实，定期组织召开应急工作会议，收集本石场安全生产事故信息，分析存在的问题，提出相应的工作措施意见。3、开展应急准备工作的指导和监督检查，督促、指导有关部门落实应急委的相关工作部署；定期向本石场应急委汇报执行情况。4、组织本石场生产安全事故应急预案的编制和修订工作，负责本石场生产安全事故应急专家组的组建和管理工作，负责本石场兼职应急队伍的管理工作，组织实施应急救援相关宣传教育和培训工作，负责汇总本石场应急保障的相关信息并及时更新。5、负责开展安全生产事故的调查处理和组织实施本石场应急能力的评估工作。
42、6、具体策划和实施本石场安全生产事故应急演练和评估考核工作。7、负责各相关部门年度应急管理经费汇总，并报应急委审核、批准。8、承办应急委交办的其他工作。3.2应急状态下的组织结构3.2.1应急指挥部在本石场范围内发生生产安全事故时，应急指挥小组根据事故类型，立即启动配套的现场处置方案进行处置。应急指挥小组的工作职责是：1、组织实施应急救援工作，全面协调生产安全事故的应急处置，调集应急资源，在本石场范围内协调现场救援所需应急力量、设备设施、信息、专家支持等。2、根据事故后果和事故态势，决定扩大应急的相关事项，批准重大应急行动。3、必要时，及时向上级部门报告，申请外部救援力量支援；与周边街道社区协
43、调，通报信息。4、审核对外发布本石场的安全生产事故信息。5、决定本级处置的生产安全事故应急救援工作的中止或终止。6、法律法规、规章规定的其他职责。3.2.2应急救援现场指挥部当发生较大事故时，本石场成立应急救援现场指挥小组（以下简称现场指挥小组）。调动一切力量，采取一切手段，以最快的速度和最有效的办法控制事态发展，其主要职责是：1、负责现场应急救援的组织实施工作。协调指挥相关部门，及时贯彻执行本石场应急指挥部布置的各项应急救援工作。2、掌握现场态势，确认救援条件，确定救援方案，根据事故发展状态，对预案实施过程中遇到的问题、发生的变化及时提出调整和处理意见。3、确定现场应急资源的需求，及时向外部
44、救援力量请求协调调配。4、组织实施伤亡人员救护、工程抢险和人员疏散、人员安置、财物转移等应急救援工作。5、记录、收集和汇总现场各类信息，及时向本石场应急指挥部及有关部门通报事故及救援情况；6、本石场生产安全事故应急指挥部交办的其他工作。3.2.3应急工作组当发生较大事故及社会影响较大的事故时，根据需要，现场指挥部一般设立4个应急工作组。根据事故类型和相应级别的不同，具体设立的专项应急工作组的数量可随之变化，具体由现场指挥部确定。各应急工作组的职责是： 综合协调组办公室负责，财务部配合，承办较大安全事故的报告，通知指挥部成员立即赶赴事故现场，联系医疗急救中心及其它各专业应急救援力量；及时向石场负
45、责人报告事故抢险救援进展情况，落实上级领导关于事故抢险救援的指示和批示。财务部牵头负责应急物资的调配和运输。 应急救援组由生产安全科负责，生产安全科科长（生产主管）兼任组长，根据应急指挥部的决策意见，指挥保卫科实施警戒；提供技术支持，参与应急监测、预警、响应、保障、善后处置工作，为救援的决策提供依据和方案；组织抢险队伍，进行抢险救援工作，发出各项具体应急指令；组织、协调相关部门及班组开展应急救援工作。 后勤保障组由办公室、财务部负责，负责组织协调相关部门准备日常应急救援物资；在发生应急情况时，组织协调有关部门，提供各种抢险救灾物资和通讯保障工作，根据需要组织有关部门保障应急所需物资、装备迅速到
46、位；负责受伤人员医疗救护，保险理赔，应急所需各类后勤组织协调工作，负责事故相关信息起草及发布等相关工作。 事故调查组由石场安全生产委员会及时介入，生产安全科科长、办公室主任负责，生产安全科科长兼任组长，具体负责事故的初步调查工作。并配合安监部门进行现场勘察取证，以及上级调查组对事故进行调查处理工作。4 预防与预警 4.1危险源监控 4.1.1由本石场安全主管牵头监督本石场危险源的监控管理工作，本石场各相关部门按照管辖原则，分别负责管理职责范围内危险源的监控管理工作。4.1.2本石场安全员、制砖焙烧车间安全员对本石场进行定期检查，做好监控检查记录。发现事故征兆按照现场处置方案及时处置，并将情况报
47、本石场领导。4.2预警行动4.2.1预警信息由本石场应急办统一接受、管理。有下列情形之一的，应立即向本石场应急委建议作出事故预警决定：1、本石场发生重大安全事故的。2、已发生的事故有可能导致其他事故发生的。3、其他单位发生的事故有可能影响本石场生产经营活动的。4、事故发生后事故级别提升或者事故影响在扩大或有可能扩大的。5、预报有重大自然灾害信息可能衍生生产安全安全事故的。4.2.2本石场应急办接到可能导致预警信息后，要密切关注事态进展，立即通知各部门负责人，报告本石场负责人和安全主管，并按照预案做好应急准备和预防工作，事态严重时，应及时上报田东县安监局安监局。4.2.3当本石场安全监控设施发出预警信息时，发现预警信息的部门、班组应按相应的应急处置预案采取相应的应对措施，并将预警信息报本石场应急办，本石场应急办接到报告后应立即查核相关情况，对相关信息分析判断，按照预案采取相应的处置措施。4.2.4事故预警决定由本石场应急指挥部发布，由本石场应急办具体承办。4.2.5当应急指挥部做出预警决定后，与突发安全生产事故预警有关的各部门、兼职应急队伍应迅速做好有关准备工作并进入待命状态。4.2.6可能导致突发安全生产事故的因素已经消除，
