31、初步设计在桥型方案的选择上做了多个方案的考虑，考虑了下承式拱桥等桥型，但因桥梁景观设置的要求，同时这些方案工程造价又太高，结合主观意见和XX交通运输厅批复，施工图采用39m+66m+39m变截面连续箱梁方案。下部结构选用了柱式圆撴，钻孔灌注桩基，重力式桥台方案。本项目起点为荣昌路与环城东路的交叉处，向北跨越XX高速公路，止于环城北路与人民东路二期之间的中点附近。设计全长0.830公里，含路基675米和大桥155米。主桥（39+66+39m）采用单箱三室变高度直腹式箱梁，梁体高度24m。桥梁与XX高速路斜交76（见跨线桥支架布置图（一）XX高速路为川渝地区经济发展的重要交通枢纽之一，日常车流量大
32、，交通不能中断。为保证现浇箱梁施工时不影响桥下高速公路的通车安全，必须设置施工安全防护通道。【2】上跨XX高速路支架总体方案简介2.1结构布置形式根据现场实际情况，我部分别在XX高速路行车道外边缘和中央隔离带外侧，沿高速路线路方向浇筑高0.5m，宽0.6m，长47.4m的C25混凝土条形基础，砼基础内预埋钢板。结合现场实际测量数据，保证高速路行车净宽度为7.5m，支架上空净高为5.5m。XX高速路横断面共设置4排3跨条形基础。基础上面采用42614mm钢管作为支架立柱，钢管顺高速路方向采用214b横向连接，将钢管四周围焊在基础预埋板上，钢管底部设置加劲肋，高速路横断面方向钢管的间距为8.35m
33、，钢管顶面布置横向分配3I56b工字钢，3I56b工字钢顶面布置I56b纵跨高速路，工字钢横向之间焊接22钢筋。钢筋上面铺设400255cm木板防护，木板要求严丝合缝，有空隙的地方用竹胶板补填。纵向I56b上布置碗扣支架2.3防护棚围护为防止高空作业人员坠落至高速公路上，横跨高速路区域箱梁部分必须采取围护措施，整个支架的四周要求形成封闭围栏。具体为在防护棚四周型钢上面竖向焊接角钢，间距为2m，横向采用16钢筋联接，高速路范围内形成四边形围栏，围栏上满挂密目安全网，安全网与木板之间不能留有空隙，用铁丝将安全网绑扎牢固，固定在围栏上面。在工字钢上翼缘焊接22钢筋，然后在上门铺设400255cm木板
34、，木板之间用抓钉固定牢固， 木板空隙处用竹胶板铺设，然后用铁钉将竹胶板与木板钉在一起，要求整个上跨高速路区域竹胶板铺设严密，不留空隙。2.4安全警示牌在施工地点两端外2Km处开始设置一块施工标志牌“前方施工请减速慢行”和“限速40”，为便上下行车辆临时分流，需将中央隔离带拆除，并留出10m长的缺口，用带有反光警示标志的锥形筒分隔高速路车道，以便浇筑条形基础混凝土和安装现浇支架。支架正对高速路处张贴警示标志“限高5.5米” “限速40”，所有支墩及钢管拄均涂刷反光警示标志。在桥梁施工区域外何处设置“前方施工的标识标牌”和高速路中央隔离带何处开口分流车辆，以及交通车辆行驶管理组织措施，请XX高速公
35、路管理局帮助制定。安全标识标牌要求如下（1）施工时，上行或下行车道全部密封,贴反光标志，采用黑白斜条纹标志，间距15cm。（2）上下行车道进出口设置防撞沙袋，两侧各设三个桶。（3）距离施工场地大约100m处设置限高5m标杆。（4）安排专职安全员24小时轮流保护标志标牌以及指挥高速路行车安全。（5）距离施工箱梁支架位置50m外设置三条减速带，强制车辆减速行驶。【3】上跨高速路主梁施工3.1施工顺序3.1.1浇筑条形基础浇筑条形基础时，需要实行交通管制，临时需要占用高速路半幅车道，以提供施工作业场地，以及堆放材料，及运输混凝土。我部将加大人员和机械设备的投入，以缩短占用车道的时间，施工时间根据现场
36、交通状况与高速公路管理单位协商决定。具体步骤如下：（1）实行交通管制，封闭全部下行车道（至泸州方向），将上行车道（至成都方向）分为两个车道，所有上下行车辆从此车道通过，然后安装模板，浇筑下行车道条形基础。最后安装钢管及工字钢支架并搭设防护棚及围栏，搭设完成后，解除下行车道（至泸州方向）的封锁。（2）待下行车道条形基础浇筑完成后，封闭全部上行车道（至成都方向），所有上下行车辆分为2个车道从上行车道（至泸州方向）通过，然后安装模板浇筑上行车道条形基础。最后安装钢管及工字钢支架并搭设防护棚及围栏。3.1.2钢管及型钢搭设待下行车道条形基础浇筑完成后，用汽车将钢管运至施工现场，然后占用全部下行车道（至
37、泸州方向）摆放吊车及堆放型钢等材料，然后将钢管吊起至安装位置焊接，要求焊缝牢固、严密。安装钢管时，我部将派2个专职安全员现场保证安全。安装钢管时实行的交通管制同上，不再重述。钢管焊接全部完成后，用216将钢管顺高速路方向连接。然后将在下行车道（至泸州方向）型钢用吊车吊至钢管顶面，人工辅助精确定位后焊接工字钢，事先在钢管上用钢筋焊接吊钩和爬梯，以便焊接操作人员系安全带和操作。工字钢铺设完成后，工字钢上翼缘焊接22钢筋，在钢筋上面铺设400255cm木板，然后用竹胶板填充木板之间的空隙，在工字钢上面焊接角钢形成围栏，然后挂密目安全网封闭施工区域。上行车道（至成都方向）工序相同，不再重述。【4】安全

42、围，施工方案和施工时间，取得相关部门的有力配合。（2）配备足够的防护人员和防护设施。（3）在进行支架搭设设时，分别在施工地点两端50米处设置一名安全员，负责过往车辆安全提示、疏导和防护。施工地点设置安全员2名，负责施工期间的安全防护工作。（4）夜间施工时设置照明设施，保证施工和行车安全，同时设置反光警示与安全标牌。（5）施工作业人员和防护人员统一穿防护衣，戴安全帽，系安全带。现场负责人和安全防护人员配置对讲机，保证信息畅通。（6）施工现场配置行政用车一台，医保人员一名，以及时应对突发事件。（7）施工期间，项目办公室电话全天24小时派人值班，保证与相关部门的联络，随时掌握并通报施工状况，积极应对
43、、协调意外事件。【5】保通期间应急预案5.1应急指挥机构成立事故应急求援“指挥领导小组”，由项目经理任组长，副经理及技术负责人任副组长，项目部其他领导及部门负责人、各队负责人、专业工程师为成员，下设应急救援办公室（设在项目部安质室，日常工作由安质室监管。）。发生事故时指挥领导小组负责全面应急救援工作。组长： XX副组长：XX成员：XX 应急领导小组办公室设联系人及电话：现场专职安全员：白天：XX，晚上：XX现场应急指挥中心有现场应急指挥一人，由施工队长担任，队长不在时由副队长担任。现场应急指挥中心人员必须是经过应急指挥训练过的人员，能做到镇定、沉着、细心、头脑冷静、反应迅速、熟悉现场。5.2各
44、部门职责指挥领导小组： （1）负责本单位“预案”的制定，修订（2）组建应急求援专业队伍，并组织实施和演练（3）检查督促做好事故的预防措施和应急救援的各项准备工作工程部：负责警戒、治安、保卫、疏散、道路管制工作。现场医疗救护及伤员分类抢救和护送转院工作由“XX县人民医院”完成。后勤组：负责抢救受伤人员的生活必需品供应及运输车辆的组织。物设部：负责抢险球员球员物资的供应和运输工作。5.3急救设施工地常被医疗急救箱。项目部配备应急救援车辆两辆：北京现代越野车、皮卡车；必要时拨打120急救电话请求救护车运送伤员。5.4医疗机构XX县人民医院（急救电话： 据施工现场约5公里）5.5救援专业队伍的组成及分
45、工项目部各职能部门和全体员工都负有事故应急救援职责，所有施工员都兼有救援组事故应急救援的骨干力量。自XX经济开发区工业园XX高速公路跨线桥连续箱梁施工至恢复XX高速交通的时间内，在距施工现场500米或200米处开始设置交通协管员提前醒过往车辆，确保车辆的行车安全。施工现场按规范要求设有安全标志，危险部位有安全警示牌。应急救援小组配备下列救援器材：医疗急救器材：担架、氧气袋、塑料袋、急救用的小药箱；抢救工具：一般工地常备用具及基本满足要求。照明器材：手电筒、应急灯、灯具等。通讯器材：电话、手机。交通工具：出应急救援车外，工地常被一辆值班面包车，该车值班24小时待命。灭火器材：日常按要求就位，经济
46、情况集中使用。工程机械：挖掘机一台，吊车一台，装载机一台。详细应急预案方案见附件：XX经济开发区工业园区XX高速跨线桥施工专项应急预案。第四章 工期保证措施为确保保通方案的顺利实施，并保证能在规定的时间内完成保通方案中所有的工作内容，在最短的时间内回复XX高速的交通，我部特制定了工期保证措施。该计划为初步拟定，待变更设计图纸下发以及高速公路管路部门审核批准后，具体工作计划再做详细的调整。XX跨线桥上部构造及路基施工工期计划 【1】主要技术措施1.1.提前技术服务，保证施工顺利进行（1）建立以项目总工为首的技术保障及决策体系，加强项目总工技术岗位负责制，行使技术质量否决权。施工中细审核、勤交底、
47、勤检查，提前做好测量、实验等各项技术工作。现场技术人员，深入一线跟班作业，搞好技术交底工作，发现问题技术解决（2）施工中积极推广运用新成果，新技术，组织专业化施工，提高施工技术水平，以科技促进工期、保工期。（3）编制详细的实施性施工组织设计，根据施组要求配齐配足施工所需的管理技术，施工资源，确保施工方案落实到位。（4）充分考虑气候因素对施工的影响，采取切实可行的冬雨季施工措施，保证施工工作作业连续进行，抓住施工有利时机，加大设备、人员投入。【2】加强材料供应，抓好物资保障选配有工作经验和管理经验的同志负责物资采购工作，加强材料计划管理，按施工计划安排材料供应。做好材料（特别是季节性）的适量储备。严把材料质量关，杜绝不达标材料进场。【3】加强机械保养，抓好设备保障选配技术素质好、责任心强的同志负责设备管理工作。加强设备的维修和保管，确保其完好率和出勤率。加强现场和工作面设备的协调使用。第五章 工程质量保证措施在施工过程中，应严格按高速公诉公路的质量标准进行控制，避免经后的重复施工。为此，项目部队各分项工程制定了行之有效的质量保证措施【1】落实质量责任制认真执行自项目经理、各级主管领导、技术负责人、质量检查、工程试
